Peroneus Insertion and Origin:

  • The Peroneus Brevis originates from the middle to distal fibula.
  • It courses down the outside of your foot.
  • It attaches to the styloid process of the fifth metatarsal.
  • if you palpate with your finger, the pain can be felt here.

Peroneus Brevis Tendon Injury Causes:

The Peroneus Brevis muscle is the primary muscle responsible for turning the foot up and out.

  • Although this motion doesn’t seem like one that would be overworked too often.
  • It is needed in almost any athletic activity to keep you balanced on your foot.
  • It is even needed to stand for long periods of time.
  • The peroneal tendons can also be damaged when you roll your ankle as they can become stretched and elongated as your foot buckles inward.
Outside of the ankle pain
Outside of the ankle, the pain usually occurs at the head of the fibula. But it can also be the subtalar joint or the peroneal tendons.

Who Is At Risk?

  • Overuse injury.
  • Standing very long periods at work.
  • Overuse during athletics.
  • Inversion ankle injury (“rolled” your ankle).
  • Injury to the subtalar joint or tarsal coalition.
  • High Arch foot.
  • If you have “clicking,” it may be a subluxed peroneus brevis.

Fibularis (Peroneus) Brevis Tendinopathy:

  • Peroneus brevis tendinopathy is essentially the same thing as peroneus brevis tenosynovitis and peroneus brevis inflammation.
  • This means an overuse or an irritation of the peroneus brevis muscle and peroneus brevis tendons.
  • If not treated properly, this could lead to a peroneus brevis split tear due to the tendon’s overuse.
  • A split tear can eventually lead to a complete tear.

Split Tear of the Peroneus Brevis Tendon:

  • Split tear of the peroneus brevis tendon is diagnosed by an MRI of the leg.
  • If it is determined that the split tear takes over more than 50% of the tendon, the tendon may be difficult to repair.
  • Initially, treatment consists of immobilizing and protecting the leg in a cast or boot. This may need to occur for at least four weeks, potentially longer.
  • This will be decided by your treating podiatrist.
  • At the same time, if the tear is significant, the decision may be made to go in surgically to repair the peroneus brevis tendon.

Progression Of Injury If No Treatment:

  • Pain worsens with overuse but initially gets better after rest when your blood gets flowing.

  • Swelling and redness on the outside of your leg, ankle, and foot.
  • The pain is palpated along the course of the tendon.
  • Pain on turning your foot up and out against tension.
  • Ruling out other problems than peroneus brevis tendonitis on X-ray.
  • Ruling out other problems through medical history.
  • If a partial tear is suspected, an MRI may be ordered if the peroneus brevis tendonitis is severe.

Peroneus Brevis Tendonitis Treatment:

1)Rest.

It may not always be possible for some athletes, but switch it up to biking or swimming for a little while!

  • No athlete wants to hear it.
  • Any high-impact activity will prolong the healing time. try to find an activity that puts less strain on the peroneus longus tendon.
  • These include swimming or biking for at least a week or two.
  • At least until the inflammation calms down.

2) Cryotherapy:

Nothing as dramatic as an ice bath, but icing for 15-20 mins will help relieve the soreness & speed healing time!

This is most beneficial after activity & soreness.
Icing 2-3x per day for about 15-20 minutes will decrease the flow of inflammatory cells into the muscle.
Afterward, the arteries will vasodilate, further flushing out inflammatory cells from the region.
Ice has deeper penetrating power than heat.
Do not use heat after activity or further irritation of the region.

Good Alternatives to Ice:

Menthol-based gels have been shown to be very effective in treating inflammation for up to 2x as long as ice.

Anti-inflammatory medication can really jump-start the healing process. Use it for 1-2 weeks until the inflammation decreases & your body can start healing.

  • These include Ibuprofen & aspirin.
  • This can be over the counter or prescription.
  • Do not take these unless instructed to do so by your physician or podiatrist.
  • Take a 1-2 week course to attempt to decrease the inflammation and speed up your healing time.
