Bunion Surgery: Cost, Recovery & Is It Worth It? 2026

Every week, patients come to my office asking whether they should have bunion surgery. It’s a significant decision — you’re talking about 6-12 weeks of recovery, real costs, and real risks. Here’s everything I tell my patients before they make that decision, including the information most surgeons don’t take enough time to cover.

When Is Bunion Surgery Actually Necessary?

Surgery is NOT indicated for cosmetic reasons alone. I recommend surgical evaluation when: pain significantly interferes with daily activities despite 3-6 months of conservative care, the bunion has progressed to where normal shoes cannot be worn, associated problems have developed (hammertoes, bursitis, skin breakdown), or the deformity is severe enough that joint damage is occurring.

Conservative treatment should always come first: wide toe box shoes, bunion splints worn at night, padding to reduce friction, and custom orthotics to reduce forces at the first metatarsophalangeal joint.

Types of Bunion Surgery

Traditional osteotomy (Austin/Chevron): A V-shaped cut in the first metatarsal bone, shifting the bone back into alignment. The most commonly performed procedure. 6-8 weeks non-weight-bearing or in a surgical boot.

Lapidus fusion: Fuses the first metatarsal-cuneiform joint, addressing the root cause of many bunions. More extensive recovery (10-12 weeks) but lower recurrence rate for hypermobile feet.

Minimally invasive bunionectomy: Small incisions, specialized instruments. Faster recovery (4-6 weeks), less soft tissue trauma. Best for mild-to-moderate deformities.

Week-by-Week Recovery Guide

  • Week 1-2: Elevation, ice, pain management. Surgical shoe or cast. Keep weight off the foot.
  • Week 3-4: Transition to walking in surgical boot for most procedures. Swelling persists.
  • Week 5-8: Progressive weight-bearing, physical therapy begins for range of motion.
  • Month 3: Most patients return to normal shoes (wide, supportive styles). Return to work for sedentary jobs.
  • Month 6: Most activities including light exercise. Athletic activity typically begins here.
  • Month 12: Full recovery. Final bone remodeling and swelling resolution complete.

Insurance covers medically necessary bunion surgery in most cases — meaning the bunion causes documented pain and functional limitation and conservative treatment has failed. Most patients rate post-op pain as 3-5/10 with proper pain management. The first 3-5 days are the most uncomfortable. Most patients transition off prescription pain medication by day 5-7.

Can bunions come back after surgery?

Recurrence rates are 5-15% for well-performed osteotomy procedures. Higher with poor footwear choices post-op. Lapidus fusion has the lowest recurrence rate (~2-3%) because it addresses the underlying joint instability.

How long after bunion surgery until I can wear normal shoes?

Most patients are in wide, supportive shoes by week 8-12. Narrow or tight dress shoes may take 6+ months as swelling fully resolves.

Is bunion surgery worth it?

For properly selected patients who have failed conservative treatment, the satisfaction rate is 85-90%. The keys are: realistic expectations, appropriate surgical selection, experienced surgeon, and commitment to the recovery protocol.
