Burning Feet: A Symptom Requiring Investigation
Burning feet — the sensation of heat, fire, or intense warmth in the feet, often worse at night or at rest — is one of the most distressing foot symptoms and one that patients often delay reporting. At Balance Foot & Ankle in Howell and Bloomfield Township, Michigan, we evaluate burning feet systematically to identify the underlying cause before directing treatment.
Peripheral Neuropathy: The Most Common Serious Cause
Peripheral neuropathy — damage to the peripheral nerves — is the leading cause of significant burning feet. The most common cause is diabetes: diabetic peripheral neuropathy follows a length-dependent pattern (longest nerves first), beginning with burning and tingling in the toes and progressing toward the ankle and calf over years. Other causes of peripheral neuropathy causing burning feet include: B12 deficiency (particularly in vegans, older adults with gastric atrophy, and metformin users who deplete B12), alcohol excess neuropathy, hypothyroidism, kidney disease uremic neuropathy, Charcot-Marie-Tooth disease, and certain medications including chemotherapy agents, metronidazole, and some antihypertensives.
Tarsal Tunnel Syndrome
Posterior tibial nerve compression at the tarsal tunnel causes burning pain in the sole — often bilateral — that is worse with prolonged standing or at night. The burning pattern of tarsal tunnel tends to be confined to the plantar surface and may have an associated electric shooting quality. A positive Tinel’s sign at the tarsal tunnel on examination provides an important clinical clue.
Athlete’s Foot
Tinea pedis (athlete’s foot) — particularly the interdigital type — causes burning, itching, and stinging between the toes and on the sole. This superficial cause of burning feet is often overlooked but is easily treated with antifungal therapy. The presence of interdigital scaling, peeling skin, or maceration is a key diagnostic feature.
Erythromelalgia
Erythromelalgia is a rare vascular condition causing episodic intense burning pain, redness, and warmth of the feet that is triggered by warmth and relieved by cold. Unlike the burning of neuropathy (which is not reliably changed by temperature), erythromelalgia burning is dramatically worsened by heat (warm rooms, exercise, warm water) and relieved by cold water immersion or fans. The condition has primary (idiopathic) and secondary (associated with myeloproliferative disorders, small fiber neuropathy) forms.

Burning feet evaluation begins with comprehensive medical history (diabetes screening, medication review, alcohol history, dietary B12 assessment), followed by neurological examination (monofilament testing, vibratory sense, reflexes). Bloodwork (fasting glucose, HbA1c, TSH, B12, B6, complete metabolic panel) identifies metabolic causes.
