Why Calluses Form on the Feet

The Risk of DIY Callus Treatment

For healthy patients, home callus treatment with a pumice stone is generally low-risk. For high-risk patients, however, home callus treatment can be dangerous. Diabetic patients who trim or debride calluses at home may not feel pain if they cut too deep, creating a wound. Patients with reduced circulation may not heal well from self-inflicted skin breaks. Medicated callus pads (like those containing salicylic acid) can cause chemical burns in diabetic and vascular patients and should generally be avoided.

Calluses — diffuse areas of thickened, hardened skin on the plantar foot and heel — form as a protective response to repetitive friction and pressure. Mild callus is normal and protective; severe callus buildup becomes painful itself, creates pressure that can cause underlying blistering and skin breakdown (particularly over metatarsal heads in diabetic patients), and in extreme cases, can crack (fissure) at the heel and create an infection entry point. The podiatric approach to callus management begins with identifying the mechanical pressure pattern driving the callus — the distribution of callus tells a biomechanical story about how the foot is loading — and then addressing both the callus itself and the underlying mechanical driver.
