Setting Realistic Expectations
Both custom orthotics and over-the-counter insoles have appropriate roles in foot care — and patients deserve honest information about when each is appropriate rather than reflexive recommendations for the more expensive option. What Over-the-Counter Insoles Do Well
Quality OTC insoles — PowerStep Pinnacle — provide generalized arch support and cushioning at accessible price points ($30-60). Research shows they reduce plantar fasciitis pain and improve comfort for mild to moderate conditions in patients without significant biomechanical complexity. For mild plantar fasciitis, mild flat feet, and general foot comfort during prolonged standing, OTC insoles are appropriate first-line interventions. Their main limitations: they are manufactured in generic shapes rather than customized to the individual foot’s specific contour and deformity pattern, they provide limited biomechanical correction, and they wear out relatively quickly under clinical-level use.
When Custom Orthotics Are Worth the Investment
Custom orthotics are fabricated from a three-dimensional mold of the individual foot (cast, scan, or foam impression) and are manufactured to specifications that address the patient’s specific biomechanical pattern. They are indicated when: OTC insoles have been tried adequately and failed; the biomechanical problem is specific and requires precise correction (significant overpronation, rigid high arch, leg length discrepancy); the condition is severe or refractory (Stage 2 PTTD, chronic plantar fasciitis, metatarsal stress fractures); the patient’s foot has significant irregularities that cannot be accommodated by generic devices; or athletic performance demands require the precise support that custom devices provide.
What the Research Shows
Multiple systematic reviews have compared custom orthotics to OTC devices for plantar fasciitis and found that custom orthotics provide superior long-term outcomes compared to flat insoles (no support), but equivalent or modestly superior outcomes compared to prefabricated insoles in short-term studies. However, most research is of short duration — custom orthotics’ advantages become more apparent over longer follow-up periods when prefabricated devices have deteriorated.

Custom orthotics cost $300-700 out of pocket; many insurance plans cover them with a prescription for a medical indication. A quality pair lasts 3-5 years with appropriate care.
