Insurance covers bunion surgery (hallux valgus correction) when it is medically necessary — meaning the bunion is causing documented pain, functional impairment, inability to wear standard footwear, nerve compression, skin breakdown, or has failed appropriate conservative treatment. Insurance does not cover bunion surgery performed purely for cosmetic reasons. Conservative Treatment Requirement
Most insurers require documentation of failed conservative treatment before approving surgical coverage. Conservative treatment typically includes: wider toe box footwear, custom orthotics, bunion pads and splints, cortisone injections for pain management, and physical therapy. We document all conservative measures and the duration of treatment in our prior authorization request. Patients who have had prior conservative treatment elsewhere should bring documentation of those attempts.
The Pre-Authorization Process
Bunion surgery requires pre-authorization from your insurer before it can be performed. Our process: we obtain your imaging (X-rays of weight-bearing feet), document your clinical findings, detail your failed conservative treatment, and submit a complete prior authorization package to your insurer. Most authorizations are approved within 3–10 business days. If denied, we assist with the appeals process. You do not need to manage this paperwork — we handle it entirely.
What You’ll Pay — Cost Breakdown by Insurance Type
Medicare (Parts A & B): After your Part B deductible ($257 in 2026), you pay 20% of Medicare-approved surgical fees. Hospital facility fees apply if surgery is in a hospital setting; our outpatient ambulatory surgery center generally results in lower out-of-pocket costs. Medigap plans cover most or all of the 20% coinsurance. Medicare Advantage: All Michigan Medicare Advantage plans cover medically necessary bunion surgery. Your cost is your specialist copay plus any deductible/coinsurance. Commercial Insurance (BCBS, Aetna, United, Cigna, HAP): Covered with pre-authorization. You’ll pay toward your deductible and the procedure applies toward your out-of-pocket maximum. Medicaid: Michigan Medicaid covers bunion surgery with medical necessity documentation.
