
Common Soft Tissue Masses of the Foot and Ankle
Discovering a lump on the foot or ankle understandably causes concern, but the majority of soft tissue masses in the foot are benign. The most common are ganglion cysts, plantar fibromas, and lipomas — each with characteristic locations, clinical presentations, and management approaches. Ganglion Cysts of the Foot and Ankle
Ganglion cysts are fluid-filled sacs arising from tendon sheaths or joint capsules. They are the most common soft tissue mass in the foot and ankle. The dorsal foot, lateral ankle, and plantar foot near the flexor tendon sheaths are common locations. Ganglion cysts appear as a smooth, round, firm to fluctuant lump that transilluminates with a bright light. They are benign but can cause pressure pain when they occur adjacent to nerves or in locations prone to shoe friction.
Small, asymptomatic ganglion cysts require no treatment. Cysts causing significant discomfort can be aspirated with a needle — aspiration provides immediate relief but recurrence rates are significant (approximately 50 percent). Surgical excision provides more reliable resolution, with recurrence rates of 5 to 10 percent when the cyst and its base attachment (the pedicle to the tendon sheath or joint capsule) are completely removed.

Plantar Fibromas
Plantar fibromas are firm, nodular growths within the plantar fascia, most commonly occurring in the medial band at the arch. They are part of the Dupuytren diathesis — a fibroproliferative disorder that also produces palmar contracture (Dupuytren contracture) in the hand. Multiple fibromas along the plantar fascia (plantar fibromatosis) can develop into a significant deformity. Small, non-painful fibromas require only observation. Growing or painful fibromas are treated with cortisone injection to reduce inflammation and slow growth. Surgical excision is reserved for symptomatic fibromas that do not respond to conservative measures, with wide local excision required to reduce high recurrence rates.
Lipomas of the Foot
Lipomas are benign fatty tumors that can occur anywhere on the foot, though the plantar fat pad is their most common location. They present as soft, mobile, painless masses. Lipomas rarely require treatment unless they reach a size that causes pain from pressure during weight bearing. Surgical excision is straightforward and curative when treatment is indicated.
When to Seek Evaluation for a Foot Mass
Any new foot mass warrants professional evaluation. Although most foot masses are benign, features that raise concern for malignancy include rapid growth, firmness or hardness inconsistent with a simple cyst, pain that is disproportionate to mass size, skin changes overlying the mass, and location deep to the plantar fascia. MRI is the imaging modality of choice for characterizing foot and ankle soft tissue masses before treatment.
