Your feet carry you through every mile of trail. In hiking, unlike most sports, foot problems can arise gradually over hours of sustained effort with limited opportunity for rest or medical care — making prevention far more important than treatment in the backcountry. Blister Prevention: The Foundation of Hiking Foot Care

Blisters are the most common hiking injury and are almost entirely preventable with proper preparation. A blister forms when friction between the skin and sock or boot creates shear stress that separates the outer skin layers, allowing fluid to accumulate in the resulting space. Once a blister forms, every subsequent step is painful and infection risk accumulates rapidly — particularly a concern in backcountry settings far from medical care.

The most effective blister prevention strategy addresses friction at its source. Moisture-wicking socks (merino wool or synthetic — never cotton) keep the skin dryer and reduce friction. Liner socks worn beneath a thicker hiking sock provide a surface for the sock layers to slide against each other rather than against the skin. Footwear that is properly fitted — with a thumb width of space at the toe, snug heel, and sufficient volume for foot swelling that occurs on long hikes — eliminates the movement that drives friction.

Identify hot spots — areas of increased warmth and rubbing — before they become blisters. Stopping to apply moleskin, athletic tape, or specialized blister prevention products (Body Glide, Trail Toes anti-chafe) at the first sign of hot spot dramatically reduces blister formation. Breaking in new boots before long hikes is essential — never take new footwear on a multi-day trip without prior break-in hiking.

If a blister forms, keep it intact whenever possible — the overlying skin provides a sterile, biologically ideal covering for the raw tissue underneath. Drain large, painful blisters that prevent walking by puncturing the base with a sterilized needle, allowing fluid to drain while keeping the overlying skin intact. Cover with moleskin with a donut cutout that eliminates direct pressure on the blister dome.

Ankle Sprains on Trail

Uneven trail terrain creates ankle sprain risk that is substantially higher than road or flat surface walking. Roots, rocks, loose scree, and unexpected terrain changes can invert or evert the ankle in an instant. Hikers carrying heavy packs have altered center of mass that increases the force of falls when the ankle gives way.

Ankle supportive footwear — mid or high-cut hiking boots — provides meaningful ankle protection on technical terrain. Lace boots firmly through the ankle portion to maintain support without overtightening the forefoot. A lace-up ankle brace worn inside the boot provides additional stability for hikers with ankle instability history. Trekking poles significantly reduce fall risk on technical terrain and provide mechanical advantage that reduces the cumulative load on the ankle per mile.

Treat trail ankle sprains immediately: PRICE protocol (Protection, Rest, Ice from a cold stream, Compression, Elevation). Do not hike on a significantly sprained ankle — seek evacuation if pain and swelling prevent weight bearing, as fractures are common with significant inversion injuries.

Toenail Injuries: Black Toenails and Loss

Subungual hematomas — bleeding beneath the toenail — occur when the toe repetitively strikes the front of the boot during downhill hiking. Boots that are too short, laces tied too loosely (allowing foot to slide forward), and long toenails all contribute. The second toe is most commonly affected because it is often longer than the great toe in many people.

Prevention requires properly sized footwear (measured with hiking socks and sized to accommodate foot swelling), tight lacing particularly through the ankle and midfoot to reduce forward foot slide, and trimming toenails short and straight across before any significant hiking. If a subungual hematoma develops and causes severe pain, trephination (draining through a small hole in the nail) provides immediate relief and can be performed in our office.

Plantar Fasciitis from Hiking

Multi-day hiking with heavy packs on hard trail surfaces creates cumulative plantar fascia loading that can trigger plantar fasciitis in predisposed individuals. Thru-hikers on the Appalachian and Pacific Crest Trails report plantar fasciitis as one of the most common injuries forcing trail departure. Symptoms typically emerge after several days of continuous high mileage.

Footwear with adequate arch support and cushioning is the primary prevention. After-hiking stretching of the calf and plantar fascia each evening significantly reduces morning symptoms. Custom orthotics designed for hiking boots provide superior arch support and load distribution for hikers with flat feet or high arches who are susceptible to plantar fasciitis.

Choosing Hiking Footwear

Footwear selection is the single most important factor in hiking foot health. Trail runners are appropriate for well-maintained trails with light to moderate loads — they offer excellent agility and ground feel with less break-in time. Traditional leather hiking boots provide superior ankle support, durability, and load-carrying capacity for technical terrain and heavy packs but require thorough break-in to avoid blisters.

Waterproof membranes (Gore-Tex) keep feet dry in wet conditions but reduce breathability, increasing sweat and blister risk in warm weather. Consider waterproofing needs versus breathability based on typical terrain and season.
