Lymphedema is chronic swelling caused by impaired lymphatic drainage, resulting in protein-rich fluid accumulation in the interstitial tissues. In the lower extremity, lymphedema affects the foot and ankle profoundly — altering skin integrity, dramatically increasing infection risk, and complicating routine podiatric care. Understanding lymphedema’s impact on foot health helps patients and caregivers manage this chronic condition more effectively.
Types of Lower Extremity Lymphedema
Primary lymphedema: Results from congenital developmental abnormalities of the lymphatic system. Milroy disease (congenital onset) and lymphedema praecox (onset at puberty, predominantly in females) are the most common forms. Primary lymphedema in the foot produces chronic bilateral (often asymmetric) swelling beginning in adolescence or young adulthood.
Secondary lymphedema: Acquired lymphatic damage from cancer treatment (most commonly breast cancer axillary node dissection or radiation, pelvic lymph node dissection for gynecologic or prostate cancer), infection (filariasis is the most common cause worldwide; cellulitis-related scarring in developed countries), trauma, or obesity. Lower extremity secondary lymphedema after pelvic or inguinal lymph node treatment is common.
How Lymphedema Affects the Foot
Skin changes: Chronic protein-rich edema produces skin fibrosis — progressive hardening and thickening of subcutaneous tissue. Advanced lymphedema produces the classic “peau d’orange” (orange peel) skin texture and eventually hyperkeratotic (papillomatous) skin changes. Skin becomes fragile, prone to fissuring, and highly susceptible to infection.
Infection risk: Lymphedematous tissue is immunologically impaired — lymphatic vessels are essential carriers of the cellular immune response. Bacterial cellulitis and dermatophyte fungal infections (tinea pedis, onychomycosis) are dramatically more common and more severe in lymphedematous limbs. A single episode of cellulitis damages lymphatic vessels, worsening underlying lymphedema — creating a destructive cycle.
Toenail changes: Chronic edema promotes toenail changes including thickening, onycholysis, and toenail fungus. Nail care is more challenging with distorted anatomy and increased infection risk.
Wound healing: Impaired lymphatic function reduces fibroblast migration and collagen synthesis, impairing wound healing — even minor skin breaks can progress to chronic wounds.
Podiatric Management in Lymphedema
Meticulous skin and nail care: Regular professional nail trimming and callus debridement prevent ingrown nails, pressure sores, and infection entry points. Moisturizing the skin daily with non-alcohol-based emollients prevents fissuring. Early recognition and treatment of tinea pedis prevents bacterial superinfection.
Compression therapy: Properly fitted compression garments (20–40 mmHg for lower extremity lymphedema) reduce edema, prevent recurrent cellulitis, and improve skin condition. Compression must be fitted by a trained therapist and replaced every 3–6 months as garments lose elasticity.
Footwear accommodation: Swollen lymphedematous feet often cannot fit standard footwear — extra-depth shoes, accommodative custom orthotics, and soft, seamless socks prevent pressure ulcers and skin injury. Medicare covers therapeutic footwear for eligible diabetic patients; patients with lymphedema and diabetes may qualify.
Infection prevention and early treatment: Any break in skin integrity requires immediate attention and consideration of antibiotic prophylaxis for patients with recurrent cellulitis.
