Podiatric involvement can precede skin manifestations by months or years, making foot symptoms an important diagnostic clue in patients who may not yet carry a rheumatologic diagnosis.

Patterns of Foot and Ankle Involvement

Psoriatic arthritis produces several distinct foot and ankle patterns. Enthesitis — inflammation at tendon and ligament insertion points — is a hallmark of PsA and most commonly affects the Achilles tendon insertion at the calcaneus and the plantar fascia origin, producing heel pain that clinically resembles but is pathologically distinct from mechanical plantar fasciitis.
