What Is the Spring Ligament?

The spring ligament — formally the plantar calcaneonavicular ligament complex — runs from the sustentaculum tali of the calcaneus to the plantar surface of the navicular. It forms the ‘floor’ of the medial longitudinal arch, directly supporting the head of the talus from below. When the spring ligament is intact and appropriately stiff, it prevents the talar head from plantar-medially migrating (the arch-collapsing movement).

The spring ligament complex has three components: the superomedial calcaneonavicular ligament (largest, most important clinically), the medioplantar oblique ligament, and the inferoplantar longitudinal ligament. MRI reliably demonstrates tears in these components.

The posterior tibial tendon (PTT) and spring ligament work as a functional unit for arch support. The PTT provides dynamic support; the spring ligament provides passive restraint. When either fails, the other is overloaded — explaining why spring ligament insufficiency and PTT dysfunction occur together in adult acquired flatfoot.

How Spring Ligament Injury Develops

Progressive degeneration rather than acute trauma is the most common mechanism. Years of increased arch strain — from obesity, flat feet, high activity levels — cause cumulative collagen fiber disruption in the spring ligament. The ligament gradually elongates and loses stiffness, allowing progressive medial arch collapse.

Acute spring ligament tears occur with sudden severe inversion or eversion injuries — often in conjunction with posterior tibial tendon injuries. A patient who rolls an ankle severely and subsequently develops progressive flat foot should be evaluated for spring ligament damage.

Risk factors: obesity (increased arch loading), age (collagen degeneration), female sex, diabetes (glycation of collagen reduces ligament integrity), inflammatory arthropathies (rheumatoid arthritis destroys periarticular connective tissue), and pre-existing flexible flat foot with already-elevated arch strain.

Diagnosis and Treatment

MRI is the gold standard for spring ligament assessment — it demonstrates disruption of the ligament fibers, thinning, increased signal (edema), and associated talar head uncovering. Weight-bearing X-rays show arch collapse, heel valgus, and medial column break.

Conservative management: custom orthotics with aggressive medial arch support and heel valgus correction — the orthotic mechanically substitutes for the spring ligament’s passive restraint. Physical therapy for tibialis posterior strengthening. Efficacious for mild to moderate spring ligament insufficiency.

Surgical treatment: spring ligament reconstruction (primary repair or allograft reconstruction) combined with PTT surgery and calcaneal osteotomy addresses all three components of adult acquired flatfoot simultaneously. This is major reconstructive surgery with 4–6 months recovery.
