What Is the Subtalar Joint?
The subtalar joint (also called the talocalcaneal joint) is the articulation between the inferior (bottom) surface of the talus bone and the superior (top) surface of the calcaneus (heel bone). It is actually a complex joint with three articular facets: the anterior, middle, and posterior facets, which together form a precise gliding mechanism.
The primary motion of the subtalar joint is inversion and eversion — tilting the foot inward or outward. This motion is critical for foot and ankle adaptation to uneven terrain, shock absorption during walking, and converting the rotational forces of the leg into the sagittal-plane motion of the foot.

Common Subtalar Joint Conditions
Subtalar Arthritis
The most common subtalar joint pathology we see in clinic. Most cases are post-traumatic — developing months to years after a calcaneus (heel bone) fracture. The joint surface is damaged at the time of fracture, leading to progressive cartilage loss and arthritis. Other causes include rheumatoid arthritis, gout, and primary osteoarthritis. Symptoms: deep heel pain, stiffness, painful instability on uneven ground.
Sinus Tarsi Syndrome
The sinus tarsi is a bony tunnel between the talus and calcaneus containing multiple ligaments, nerves, and blood vessels. Sinus tarsi syndrome occurs when these structures are injured (typically from a lateral ankle sprain) causing chronic pain, instability, and a feeling of ‘giving way’ in the lateral hindfoot. It’s one of the most commonly missed diagnoses following ankle sprains.
Tarsal Coalition
A tarsal coalition is an abnormal bony or fibrous bridge between two hindfoot bones — most commonly the calcaneonavicular or talocalcaneal coalition. Present from birth, it typically becomes symptomatic in adolescence when the coalition ossifies. Symptoms: painful flatfoot, rigid heel, recurrent ankle sprains, and inability to move the hindfoot through a normal range.

- CT scan — the best imaging for subtalar arthritis, tarsal coalition, and bony detail of the calcaneal fracture malunion
- MRI — soft tissue detail for sinus tarsi syndrome, ligament status, and early cartilage changes
- Diagnostic injection — injection of local anesthetic into the subtalar joint under fluoroscopic or ultrasound guidance; if it relieves pain, the joint is confirmed as the source
Subtalar Joint Treatment
Conservative Management
- Activity modification — avoid uneven terrain and high-impact activities temporarily
- Custom orthotics — control subtalar pronation and reduce joint loading; highly effective for early arthritis
- Ankle-foot orthosis (AFO) — for significant instability or moderate-to-severe arthritis
- Corticosteroid injection — targeted subtalar joint injection provides significant relief for 2–6 months
- Physical therapy — proprioception, peroneal strengthening, gait training
Surgical Treatment: Subtalar Arthrodesis
When conservative treatment fails for subtalar arthritis (typically after 6+ months), subtalar fusion (arthrodesis) is the definitive treatment. The joint surfaces are removed and the talus and calcaneus are held together with screws until they fuse into one bone.
