Venous Insufficiency — The Most Common Cause

The resulting hydrostatic pressure forces fluid out of the capillaries into the surrounding tissue, producing the characteristic pitting edema (fluid that leaves an indent when pressed) that accumulates through the day and is at its worst by evening.

The cardinal features of venous insufficiency edema: bilateral (both ankles), begins by midday and worsens through the afternoon, significantly better in the morning after overnight leg elevation, associated with a feeling of heaviness or fatigue in the legs (but typically not sharp pain), and worsens significantly with prolonged standing or sitting. Over time, CVI can produce skin changes — brown hemosiderin deposits, lipodermatosclerosis (skin thickening), and ultimately venous stasis ulcers — particularly at the medial ankle. Early intervention with compression and activity modification prevents these complications.

Lymphedema

Lymphedema is swelling caused by impaired lymphatic drainage — the lymph vessels that normally carry excess tissue fluid back to the circulation are damaged, blocked, or absent. Unlike venous edema, lymphedema does not fully resolve with overnight elevation, and the swelling is non-pitting (the fluid becomes protein-rich and fibrous over time). Primary lymphedema develops from lymphatic developmental abnormalities and often presents at puberty. Secondary lymphedema is more common and develops after lymph node dissection (cancer surgery), radiation, recurrent cellulitis, or filariasis (in endemic regions).

Lymphedema requires specialized management — complete decongestive therapy (CDT), which combines manual lymphatic drainage massage, compression bandaging, and specific exercise protocols. Medication-Induced Ankle Swelling

Many common medications cause bilateral painless ankle swelling as a side effect by altering capillary permeability, fluid balance, or venous tone. The most common offenders are calcium channel blockers (amlodipine, nifedipine, felodipine — among the most prescribed antihypertensive medications), which cause peripheral vasodilation and fluid leakage into tissue. Other common drug causes include NSAIDs (water retention), corticosteroids, hormonal contraceptives and hormone replacement therapy, gabapentin and pregabalin, and some diabetes medications (thiazolidinediones like pioglitazone).

If your ankle swelling began within weeks of starting a new medication, medication-induced edema is a strong possibility. Do not stop the medication without consulting your prescribing physician — there are often alternative medications with lower edema risk within the same drug class. Calcium channel blocker-induced edema, for example, can often be reduced by switching to a different class of antihypertensive or by adding an ACE inhibitor.

Systemic Conditions Causing Ankle Swelling

Bilateral painless ankle swelling is a classic physical finding in three major systemic conditions: congestive heart failure (where reduced cardiac output increases venous backpressure), nephrotic syndrome (kidney disease causing protein loss in urine and reduced plasma oncotic pressure), and liver cirrhosis (impaired albumin synthesis reducing plasma oncotic pressure). DVT — When Painless Ankle Swelling Is Dangerous

Deep vein thrombosis (DVT) — a blood clot in the deep veins of the calf or thigh — is commonly assumed to always cause pain. In reality, up to 50% of DVT cases present with swelling as the only symptom, without significant pain, redness, or warmth. This makes DVT a critical concern for any new unilateral ankle or calf swelling without an obvious cause, particularly when it develops after: prolonged immobility (long-haul flight, bed rest, post-surgical recovery), starting hormonal contraceptives or hormone replacement, recent cancer diagnosis, or in patients with known clotting disorders.
