What is granuloma annulare and what does it look like?
Granuloma annulare (GA) is a benign inflammatory skin condition characterized by ring-shaped (annular) or arc-shaped raised bumps or rashes. The lesions have flesh-colored, red, or purple firm papules arranged in a circle or semicircle, typically 1–5 cm in diameter. Most commonly appears on the hands, feet, ankles, and lower legs. The localized form (single or few lesions) is most common. The generalized form involves widespread lesions and is more associated with diabetes and thyroid disease. GA is often confused with ringworm (tinea corporis) but unlike fungal infections, it doesn't cause scaling and doesn't respond to antifungals. What causes granuloma annulare?
The exact cause of granuloma annulare is not fully understood, but it represents a delayed hypersensitivity reaction with inflammatory cell infiltration and collagen degeneration in the dermis. Associated factors include: diabetes mellitus (especially generalized GA), thyroid disease, malignancy (rarely — particularly with generalized form), sun exposure (actinic GA), insect bites or minor trauma triggering localized lesions, and certain medications. Most cases of localized GA in otherwise healthy individuals have no identifiable trigger and are considered idiopathic. The association with diabetes is important — generalized granuloma annulare may be a cutaneous marker of underlying or undiagnosed diabetes.
How is granuloma annulare treated?
Localized GA often resolves spontaneously within 1–2 years without treatment — most dermatologists recommend watchful waiting for asymptomatic localized GA. When treatment is desired: potent topical corticosteroids (applied twice daily under occlusion), intralesional cortisone injections, cryotherapy (liquid nitrogen), laser treatment (Nd:YAG), and narrow-band UVB phototherapy for widespread GA. For generalized GA associated with an underlying condition, treating the underlying disease (diabetes, thyroid disorder) may improve skin lesions. Systemic treatments (hydroxychloroquine, dapsone, isotretinoin) are reserved for severe widespread cases. A podiatrist or dermatologist can confirm the diagnosis and recommend appropriate management.
Is granuloma annulare on the feet related to diabetes?
The localized form of GA has a weak association with diabetes, but generalized granuloma annulare (widespread lesions) has a well-documented association with diabetes mellitus, thyroid disease, and dyslipidemia. Multiple studies show that patients with generalized GA have significantly higher rates of diabetes compared to controls. When generalized GA is diagnosed, screening for diabetes (fasting glucose, HbA1c) is recommended. Conversely, diabetic patients may develop GA as one of several cutaneous manifestations of their metabolic condition.
