Ankle Arthroscopy Michigan

Ankle arthroscopy is a minimally invasive surgical procedure that uses a small camera (arthroscope) and specialized instruments through two or three small portals to diagnose and treat conditions inside the ankle joint. It avoids large incisions, allows direct visualization of the cartilage and ligaments, and offers faster recovery than open ankle surgery. Dr. Tom Biernacki DPM at Balance Foot & Ankle performs ankle arthroscopy in Michigan at our Howell, Brighton, and Bloomfield Hills locations.

Conditions Treated with Ankle Arthroscopy

  • Osteochondral lesions of the talus (OLT): Arthroscopic microfracture, debridement, or OATS cartilage transplantation
  • Anterior or posterior ankle impingement: Removal of painful bone spurs (osteophytes) from the tibiotalar joint margin
  • Loose bodies: Removal of bone or cartilage fragments floating in the joint
  • Ankle synovitis: Removal of inflamed synovial tissue
  • Arthroscopic ankle fusion: Minimally invasive preparation and screw fixation for end-stage arthritis
  • Sinus tarsi debridement: Treatment of sinus tarsi syndrome
  • Peroneal tendon pathology: Tendon repair, subluxation correction
  • Lateral ligament reconstruction: Arthroscopic-assisted Brostrom procedure

The Ankle Arthroscopy Procedure

Ankle arthroscopy is performed as an outpatient procedure under ankle block (regional anesthesia) with sedation or general anesthesia. Two anteromedial and anterolateral portals (each ~5mm) provide access to the ankle joint interior. A small-diameter (2.7–3.5mm) arthroscope transmits magnified HD video images. Specialized 3–4mm instruments perform the procedure under direct visualization. Posterior portals are used for posterior impingement and posterior ankle pathology. Total procedure time is typically 30–60 minutes.

Recovery

Recovery after ankle arthroscopy depends on the specific procedure performed. Most diagnostic arthroscopy and simple synovectomy/loose body removal allows immediate full weight-bearing in a lace-up boot within 1 week. Microfracture for OLT requires 6 weeks non-weight-bearing to protect the fibrocartilage healing. Arthroscopic fusion requires 8 weeks non-weight-bearing. All patients begin range-of-motion exercises early and progress to physical therapy at 2–6 weeks.

Advantages Over Open Surgery

  • Two 5mm portals instead of 5–10cm open incision
  • Less soft tissue trauma and post-operative pain
  • Faster return to activity for simple procedures
  • Lower wound complication risk
  • Simultaneous diagnostic survey of the full joint

Frequently Asked Questions

Is ankle arthroscopy painful?

Ankle arthroscopy is performed under regional or general anesthesia — the procedure itself is painless. Post-operative pain is typically mild to moderate for the first 3–5 days and is well-managed with oral medications. Most patients are surprised by how comfortable recovery is compared to open surgery.

Is ankle arthroscopy covered by insurance?

Ankle arthroscopy for documented pathology (OLT, loose bodies, impingement, synovitis) is covered by most Michigan insurance plans including Blue Cross Blue Shield, Aetna, and Medicare when indicated. Our team handles prior authorization and insurance verification.
