The extensor digitorum longus lifts the lesser toes; the extensor hallucis longus lifts the big toe; the tibialis anterior is the major ankle dorsiflexor. These tendons pass under the extensor retinaculum — a fibrous band across the front of the ankle — before branching to their respective toes.

Pain along these tendons at specific locations helps identify which structure is involved. Pain at the front of the ankle suggests retinacular irritation. Pain along the midfoot dorsum at the lace zone is classic extensor tendonitis. Pain at a specific toe joint suggests extensor hood involvement or MTP synovitis.

Common Causes

Tight shoe lacing is the most common cause — runners who lace too tightly compress the extensor tendons against the navicular bone. Rapid training volume increase strains the extensor tendons through overuse. Hill running and excessive stair climbing repetitively load the tibialis anterior during ankle dorsiflexion under load. Direct trauma (impact on the dorsum) can cause acute tendon inflammation.

Flatfoot overpronation can impinge extensor tendons as the midfoot bone structure shifts. Footwear with rigid tongue pressing on the dorsum during activity recreates the compression mechanism with every step.

Addressing footwear is first priority: window lacing (skipping the eyelet over the painful area) eliminates compression. Activity modification to reduce tendon loading during healing. Ice and NSAIDs manage acute inflammation. Eccentric strengthening and calf stretching reduce the compensatory toe-gripping that overloads extensors.
