How do I know if my child has Sever's disease vs a stress fracture?

Sever's disease pain is usually bilateral (both heels), worse at the start of activity, and improves with warm-up. Pinching the sides of the heel bone reproduces the pain. Stress fractures are almost always unilateral, get worse with activity, and don't respond to heel cushions. If pain is one-sided or worsens over 10 days of treatment, get an X-ray.

Does my child need to stop sports?

No. Research supports activity modification rather than full rest — full cessation actually delays recovery. The sweet spot is 50% practice volume with immediate icing after, plus heel cushions in cleats and regular shoes. Complete rest is reserved for pain at rest (not just with activity).

When will it go away?

Sever's disease resolves completely when the calcaneal growth plate closes (ages 14-16 for most kids). Until then, symptoms may come and go with growth spurts and higher activity. 95% of patients we treat have zero long-term effects.
