Intra-articular calcaneal fractures — constituting 60–75% of all calcaneal fractures — are among the most debilitating and surgically challenging injuries in foot and ankle surgery. The calcaneus is the largest tarsal bone and the foundation of the hindfoot; its fracture from axial loading (typically fall from height or motor vehicle accident) disrupts the posterior facet of the subtalar joint and produces a characteristic flattened, widened, varus-deformed hindfoot with posterior facet incongruity. Management decisions remain among the most contested in orthopedic trauma.

Sanders CT Classification
The Sanders classification — based on the number and location of fracture lines crossing the posterior facet on coronal CT imaging — guides surgical decision-making. Sanders Type I: non-displaced fractures regardless of number of fracture lines — treated non-operatively with excellent outcomes. Type II: a single fracture line dividing the posterior facet into two fragments (IIA, IIB, IIC based on location) — ORIF with excellent to good outcomes in 70–80% of cases. Type III: two fracture lines producing three posterior facet fragments — ORIF with satisfactory outcomes in 60–70%; some Type IIIAC fractures with central depression are particularly challenging. Type IV: four or more fragments producing comminution of the entire posterior facet — ORIF has poor outcomes; primary subtalar fusion may be considered.
ORIF vs. Non-Operative Management
The landmark ORIF vs. non-operative trial (Buckley et al., JBJS 2002) failed to demonstrate a significant overall benefit of ORIF over non-operative management at 2 years — but subgroup analysis revealed significant benefit of ORIF in patients with: non-workers’ compensation claims, Sanders Type II and III fractures in patients under 29 years old, lighter body weight, and female sex. A subsequent long-term follow-up study demonstrated that ORIF patients had significantly better outcomes at 10-year follow-up compared to non-operative management. The consensus has evolved toward ORIF for displaced intra-articular fractures in physiologically young, active patients with Sanders Type II–III fractures, adequate soft tissue condition (wound closure requires soft tissue recovery from fracture blistering — typically 7–14 days of delay), and no diabetes or smoking that would preclude surgical wound healing. Primary subtalar fusion at the time of ORIF is considered for Sanders Type IV fractures.
