What Is Crossover Toe?
Crossover toe — sometimes called second toe dislocation or floating toe — is a condition in which the second toe progressively drifts medially (toward the big toe) and eventually crosses over it, producing a painful, visually distinctive deformity. The underlying cause is disruption of the plantar plate — a thick fibrocartilaginous structure that forms the floor of the second metatarsophalangeal (MTP) joint capsule and provides the primary restraint against upward and medial dislocation of the toe. When the plantar plate fails, the stabilizing forces of the surrounding structures pull the toe into its characteristic crossover position.
Why the Second Toe Is Most Affected
The second toe is the most common site for plantar plate disruption and crossover deformity for two reasons. First, when the first metatarsal is shortened or elevated — as in hallux valgus (bunion) or post-bunion surgery — more weight transfers to the second metatarsal head, increasing the load on the second MTP joint and its plantar plate. Second, the second toe is often the longest toe, making it the most vulnerable to compression and dorsal subluxation forces from tight footwear. Patients with uncorrected bunions often develop second toe crossover deformity as a secondary consequence of the altered forefoot mechanics.
Staging and Symptoms
Plantar plate tears are classified from Stage 0 (pain without deformity) through Stage 4 (complete dislocation with toe crossing over the adjacent toe). Early stages produce forefoot pain under the second metatarsal head, warmth and swelling at the MTP joint, and subtle malalignment when observed from the front. Advanced stages are visually obvious and may prevent shoe wear entirely. The ‘drawer test’ — applying upward and lateral pressure to the second MTP joint to assess instability relative to the first MTP joint — is the primary clinical test for plantar plate disruption.
Conservative Treatment for Early Stages
Stage 0 and 1 lesions can sometimes be managed conservatively. Taping the affected toe in a plantar-flexed, medially deviated position offloads the plantar plate and holds the joint in reduced alignment. Metatarsal pads proximal to the metatarsal heads reduce forefoot pressure. Wider footwear reduces the medial compression forces on the toe. Conservative care rarely reverses the deformity but can halt progression in early stages. Corticosteroid injection into the MTP joint provides anti-inflammatory relief but is used cautiously as repeated injection may weaken the plantar plate.
Surgical Repair: Plantar Plate Reconstruction
Stage 2 and beyond with functional limitation require surgical intervention. Plantar plate repair restores the disrupted plantar capsular structure through a dorsal approach to the MTP joint. The joint is distracted to expose the plantar surface, and the torn plantar plate is repaired using suture anchors placed into the metatarsal head with the suture tails passed through the plantar plate tissue for secure reapproximation. If the plate is too attenuated for direct repair, a tissue augmentation graft may be used. Simultaneously, extensor tendon lengthening and dorsal MTP capsular release correct the dorsiflexion deformity, and any bunion deformity that is contributing to second MTP overload is corrected at the same procedure. A Weil shortening osteotomy of the second metatarsal reduces the lever arm that caused the plantar plate failure.
Crossover toe deformity is progressive and becomes increasingly difficult to correct in advanced stages. Early evaluation and treatment provide the best outcomes. Not every case of plantar plate tear is straightforward. In our clinic we routinely rule out three look-alike conditions before confirming the diagnosis. If your symptoms don’t match the classic presentation, one of these may explain the pain — which is why physical exam matters more than self-diagnosis.

Plantar plate tear is one of the most missed diagnoses in forefoot pain. Patients come in saying ‘metatarsalgia’ but in our clinic we check the drawer test at the 2nd MTP — if the toe lifts easily, the plantar plate is compromised. Early stage responds beautifully to a metatarsal pad placed BEHIND (not under) the metatarsal head, a stiff-soled shoe, and taping the toe down. Ignored plantar plate tears progress to a ‘floating toe’ and eventual crossover toe deformity. Dr. Biernacki emphasizes early intervention — a tear caught in month 2 rarely needs surgery; caught in year 2, it almost always does.
