Yes, Medicare covers toenail trimming — but only for patients who have specific qualifying medical conditions that make professional nail care medically necessary. If you’ve been putting off nail care because you’re worried about the cost, this post will tell you exactly what you need to know.
Why Toenail Care Is a Medical Necessity for Some Patients
For healthy adults, trimming toenails is a routine self-care task. But for patients with diabetes, neuropathy, vascular disease, or other systemic conditions, overgrown or improperly trimmed toenails can cause serious medical problems:
- Thickened, curled nails create pressure sores that become infected wounds in diabetic patients
- Ingrown toenails in patients with poor circulation heal slowly and can become limb-threatening infections
- Patients with neuropathy can’t feel nail pressure and pain until damage is already done
- Patients on blood thinners face bleeding risk with amateur nail trimming
For these patients, professional podiatric nail care isn’t a luxury — it’s limb preservation.
Qualifying Conditions for Medicare Nail Care Coverage
Medicare covers routine nail care (trimming, debridement of thick nails, callus removal) every 61 days for patients with one or more of the following documented conditions:
- Diabetes mellitus — with associated neuropathy, vascular changes, or poor wound healing
- Peripheral neuropathy — nerve damage causing loss of sensation
- Peripheral artery disease — reduced blood flow to the feet
- Chronic venous insufficiency — venous disease affecting leg and foot circulation
- Arteriosclerosis obliterans
- Buerger’s disease
- Severe chronic renal disease
The Documentation Requirement
To bill Medicare for routine nail care, your podiatrist must have documentation of your qualifying condition — either from their own records or from a visit with your primary care physician or specialist within the previous 6 months. This is sometimes called a “Class Findings” note.

How Often Does Medicare Cover Nail Care?
Medicare covers routine nail debridement every 61 days (approximately every 2 months) for qualifying patients. Some patients with highly active conditions may qualify for more frequent visits with additional documentation.
What You Pay
After meeting your Part B deductible ($257 in 2026), Medicare pays 80% of the approved amount. You pay 20% — or nothing, if you have a Medigap (Medicare supplement) policy that covers coinsurance. Most patients with qualifying conditions pay $0-$15 for a nail care visit at our office.
