The combination of barefoot training, explosive lateral movement, and sustained ground grappling creates injury patterns unique to these disciplines. Common Foot and Ankle Injuries in Combat Sports
Ankle Sprains from Takedowns and Throws
Wrestlers, judokas, and BJJ practitioners are frequently thrown or taken down with the foot planted in awkward positions. Lateral ankle sprains — and less commonly, high ankle sprains — result from the sudden inversion or external rotation forces applied during these maneuvers. High ankle sprains involving the syndesmotic ligament complex require longer recovery than standard lateral ankle sprains and should be evaluated with weight-bearing X-ray to rule out associated fibular fracture.
Turf Toe and Big Toe Sprains
Grappling from the knees — a common BJJ and wrestling position — loads the dorsiflexed big toe against the mat. Sudden pushing off from this position can hyperextend the first metatarsophalangeal joint, producing a turf toe injury. These ligamentous sprains range from mild (Grade I) to complete plantar plate rupture (Grade III). Grade III injuries may require surgical repair and extended non-weight-bearing. Even Grade I and II injuries require protective management to prevent chronic joint instability.
Stress Fractures from High Training Volume
Combat sport athletes who train twice daily in competitive preparation periods often develop metatarsal stress fractures from the combination of barefoot floor training and repetitive impact from kicking and footwork drills. The second and third metatarsals are most commonly affected. Pain that worsens through training sessions and does not fully resolve with rest warrants imaging — MRI detects stress fractures earlier than plain X-ray.
Plantar Skin Infections
Shared training mats are a recognized vector for tinea pedis (athlete foot), plantar warts, and occasionally more serious bacterial skin infections. Wearing flip flops in locker rooms and showers, inspecting the feet regularly, and treating early infections promptly prevent escalation. Plantar warts on weight-bearing surfaces are particularly problematic for combat athletes who train barefoot — early treatment prevents spread and eliminates the chronic pain that alters training mechanics.
Black Toenails from Striking Disciplines
Muay Thai, kickboxing, and karate practitioners who kick heavy bags and pads repeatedly develop subungual hematomas — blood pooling beneath the toenail — from repeated toenail contact. Proper foot position during kicking and trimming toenails short prevents most cases. Painful acute hematomas can be decompressed in the office for immediate relief.
Footwear and Protection Considerations
Most combat sports require barefoot or minimally shod training, limiting protective options during practice. Ankle braces worn under wrestling shoes or MMA shorts provide lateral support without violating training rules. Between sessions, supportive athletic footwear and — when indicated — custom orthotics worn during conditioning work protect against cumulative foot stress outside the training floor.

NOT a true biomechanical orthotic — this is a cushion insole. But for patients who want gel pressure relief instead of arch correction (or to add ON TOP of factory insoles in work boots), this is the best gel option on Amazon.
