Foot pain is one of the most common musculoskeletal complaints of pregnancy, affecting up to 75% of pregnant women at some point during their three trimesters. While many of these changes are physiological and self-limiting, certain foot conditions that develop or worsen during pregnancy benefit from early treatment — both for immediate comfort and to prevent changes that persist postpartum.

Normal Foot Changes During Pregnancy

Several foot changes during pregnancy are physiological — expected and related to the hormonal and biomechanical changes of pregnancy:

  • Edema (swelling): Dependent edema of the ankles and feet is nearly universal in the third trimester, resulting from increased blood volume, compression of pelvic veins by the enlarging uterus, and reduced venous return. Mild bilateral symmetric ankle swelling that improves overnight with leg elevation is normal.
  • Foot flattening and lengthening: Relaxin — the hormone that loosens pelvic ligaments for delivery — simultaneously loosens foot and ankle ligaments throughout pregnancy. This produces measurable foot lengthening (average 2–3mm) and arch flattening. Studies show this change persists postpartum in approximately 50% of women, representing a permanent increase in shoe size after the first pregnancy.
  • Increased foot temperature: Increased circulation and peripheral vasodilation causes feet to feel warmer than usual — normal in pregnancy.

Plantar Fasciitis During Pregnancy

Plantar fasciitis is the most common podiatric complaint during pregnancy. The combination of weight gain (increasing plantar fascial load), ligamentous laxity from relaxin (reducing arch support), and altered gait from the shifting center of gravity creates the perfect storm for plantar fascia overload. The characteristic sharp morning heel pain develops progressively, often in the second and third trimesters.

Safe treatment options during pregnancy include:

  • Custom orthotics with medial arch support — the most effective single intervention, suitable throughout pregnancy
  • Supportive footwear — avoiding flat sandals and bare feet on hard surfaces
  • Calf and Achilles stretching program
  • Night splints to maintain dorsiflexion overnight
  • Acetaminophen for pain relief (NSAIDs are avoided during pregnancy, particularly third trimester)
  • Physical therapy for stretching and strengthening

Cortisone injections are generally avoided during pregnancy; PRP is similarly deferred. Most plantar fasciitis cases respond to conservative management during pregnancy.

Warning Signs That Are Not Normal

Foot swelling during pregnancy that requires urgent medical evaluation (not just podiatric care):

  • Sudden severe unilateral leg swelling: Asymmetric swelling of one leg significantly worse than the other raises concern for deep vein thrombosis (DVT) — a pregnancy-related clotting risk. Requires urgent evaluation with Doppler ultrasound.
  • Swelling with severe headache, visual changes, or upper abdominal pain: May indicate preeclampsia — requires immediate obstetric evaluation.
  • Swelling that is red, hot, and tender: Cellulitis or thrombophlebitis — requires same-day medical evaluation.

Postpartum Foot Changes

Many women notice their feet remain larger after pregnancy. This is expected — the permanent arch flattening and foot lengthening from relaxin-induced ligament laxity during pregnancy frequently does not fully reverse. Custom orthotics that support the new arch structure and appropriate shoe sizing (up half a size or full size for many women) are the appropriate long-term management. Postpartum plantar fasciitis may persist for months after delivery, particularly if the new arch structure is unsupported.
