Peripheral arterial disease (PAD) — atherosclerotic occlusion or stenosis of the arteries supplying the lower extremities — is present in approximately 15% of patients over 70 and is the most common reason that foot and ankle wounds fail to heal despite optimal local wound care. Recognizing PAD as a contributing factor to impaired wound healing, initiating appropriate vascular assessment, and coordinating revascularization with vascular surgery when indicated is one of the most important roles of the podiatric surgeon in diabetic foot wound care.
Ankle-Brachial Index (ABI) and Vascular Assessment
The ankle-brachial index (ABI) is the ratio of systolic blood pressure at the ankle to systolic blood pressure at the arm — the fundamental non-invasive screening test for PAD: ABI >1.3 (non-compressible calcified vessels — falsely elevated in diabetics; requires toe-brachial index instead); ABI 1.0–1.3: normal; ABI 0.9–1.0: borderline; ABI 0.7–0.9: mild PAD; ABI 0.5–0.7: moderate PAD; ABI <0.5: severe PAD — critical limb ischemia threshold. Toe-brachial index (TBI): more accurate than ABI in diabetic patients with calcified vessels — TBI <0.6 indicates significant toe ischemia; transcutaneous oxygen pressure (TcPO2): >40 mmHg required for wound healing — values below 20–25 mmHg indicate severe ischemia with very poor healing potential. Referral criteria: ABI <0.6, non-healing wound, or rest pain warrants referral to vascular surgery for duplex ultrasound and angiography to assess revascularization candidacy.
Revascularization and Wound Healing
Percutaneous transluminal angioplasty (PTA) and stenting: endovascular revascularization for appropriate arterial anatomy (tibial artery angioplasty for below-knee PAD); avoids surgical incision; preferred for high-risk surgical patients; restenosis rates vary by vessel diameter and lesion length. Surgical bypass: femoral-popliteal or femoral-tibial bypass for long-segment occlusions not amenable to endovascular treatment; excellent long-term patency with autologous vein graft; requires adequate inflow and outflow vessels. Wound healing after revascularization: adequately revascularized wounds have equivalent healing potential to wounds in non-ischemic patients; revascularization before aggressive wound debridement prevents ischemic wound expansion.
