Most patients underestimate how much the post-operative phase determines Toe Fusion : Hammertoe & Hallux Rigidus Arthrodesis outcomes — not the surgery itself. Our podiatric surgeons identify the single recovery variable that separates patients who return to full activity on schedule from those who experience setbacks. Toe fusion surgery (arthrodesis) permanently fuses a toe joint to eliminate pain from arthritis, severe hammertoe, or hallux rigidus. It is one of the most reliable procedures in foot surgery — but requires careful patient selection and a 6–10 week recovery. Balance Foot & Ankle performs toe fusion procedures in Howell and Bloomfield Hills, MI.
Types of Toe Fusion Surgery
| Fusion Type | Indication | Joint Fused | Fixation |
|---|---|---|---|
| 1st MTP arthrodesis (hallux rigidus) | End-stage 1st MTP arthritis; failed conservative care; Grade 3–4 hallux rigidus | Great toe MTP joint | Dorsal plate + compression screw; or crossed screws |
| PIP joint arthrodesis (hammertoe repair) | Rigid hammertoe of lesser toes with fixed deformity at PIP joint | Proximal interphalangeal joint | K-wire (removed at 6 weeks) or implant (permanent) |
| DIP joint arthrodesis | Mallet toe deformity; DIP arthritis | Distal interphalangeal joint | K-wire or small fusion implant |
| 5th toe PIP fusion | Rigid 5th toe hammertoe; soft corn | 5th toe PIP | K-wire or implant |
1st MTP Fusion (Hallux Rigidus) — Detailed Recovery
| Phase | Timing | Activity |
|---|---|---|
| Acute | Weeks 0–2 | NWB; posterior splint; strict elevation |
| Early healing | Weeks 2–6 | Heel-only weight-bearing in stiff post-op shoe; suture removal ~14 days |
| Consolidation | Weeks 6–10 | Progressive weight-bearing; X-ray confirms fusion at 8–10 weeks |
| Rehabilitation | Months 3–4 | Transition to wide shoe with stiff sole; PT for gait retraining |
| Full activity | Months 4–6 | Normal shoes (must accommodate fused toe in plantargrade position) |
Hammertoe PIP Fusion — Recovery
| Phase | Timing | Notes |
|---|---|---|
| Post-op | Week 0–2 | Flat weight-bearing in surgical shoe; K-wire or implant in place; buddy tape toe |
| Healing | Weeks 2–6 | X-ray at 6 weeks; K-wire removal if used (in-office; takes 30 seconds) |
| Shoe transition | Weeks 6–8 | Wide-toe-box sneaker; avoid tight toebox |
| Full recovery | Months 3–4 | Swelling resolves; final toe position stable |
K-Wire vs. Implant for Hammertoe Fusion
| Factor | K-Wire | Fusion Implant (Smart Toe, etc.) |
|---|---|---|
| Removal required? | Yes — in office at 6 weeks | No — permanent |
| Infection risk | Slightly higher (wire exits skin) | Lower (fully implanted) |
| Cost | Lower | Higher (implant cost) |
| Patient comfort | Wire protrudes from toe tip; must avoid catching | No external hardware; more comfortable |
| Fusion rate | Comparable | Comparable; some evidence for improved alignment |
What to Expect After 1st MTP Fusion
- Great toe will not bend: the 1st MTP joint is permanently rigid; this is the intent of the surgery — patients adapt within months and walk normally
- Shoes must accommodate: the toe is fused at ~10–15° dorsiflexion to allow normal gait; most standard shoes accommodate this well; high heels are not compatible
- Pain relief is high: 90–95% patient satisfaction rates for 1st MTP fusion; it is the gold standard for hallux rigidus Grade 3–4
- Gait adapts: patients learn to push off through the 1st ray naturally; runners return to running in 4–6 months; non-union rate ~5%

Doctor Answer
What is toe fusion surgery and what is the recovery?
Toe fusion (interphalangeal arthrodesis) permanently straightens a rigid hammertoe or claw toe by fusing the proximal or distal interphalangeal joint in a corrected position using a pin, screw, or implant. It provides durable correction compared to soft tissue procedures that have higher recurrence rates. Recovery involves wearing a surgical shoe for 4-6 weeks while the bone fusion consolidates. The pin (if used) is removed at 4-6 weeks in-office. Full healing takes 6-8 weeks, with most patients returning to regular footwear at that point.
