What Is Freiberg’s Disease?
Freiberg’s disease (Freiberg infraction) is avascular necrosis of a metatarsal head — most commonly the second, occasionally the third or fourth. Reduced blood supply causes the metatarsal head to collapse, fragment, and deform, leading to forefoot pain and MTP joint dysfunction.
It most often affects females between ages 10–18, though it can present in adults of any age. A long second metatarsal is a risk factor.
Symptoms
Pain, swelling, and tenderness over the affected MTP joint at the ball of the foot. Pain worsens with toe-off phase of walking and high heels. Limited range of motion develops as the joint degenerates. Some patients feel a catching or clicking sensation in the joint.
Diagnosis
Weight-bearing foot X-rays show metatarsal head flattening, sclerosis, and fragmentation — graded on the Smillie classification system (Stages I–V). MRI detects early avascular changes before X-ray findings appear and helps guide treatment decisions.
Staging and Treatment
Stages I–III (Early: Revascularization Possible)
Conservative treatment: offloading with metatarsal pads behind the affected head, rocker-bottom shoes, custom orthotics with metatarsal bar. Activity modification. Short-term cast immobilization for acute pain. This phase aims to protect the joint while natural revascularization occurs.
Stages IV–V (Advanced: Significant Joint Damage)
Conservative care still the first line. Surgical options include: joint debridement (synovectomy, loose body removal), dorsiflexion osteotomy of the metatarsal head (rotates damaged surface away from load-bearing area), and metatarsal shortening osteotomy. Joint resection arthroplasty or implant in severe cases.
Prognosis
Early-stage disease treated conservatively has good outcomes. Advanced disease with significant joint collapse tends to cause persistent symptoms. Surgical outcomes are generally favorable — most patients achieve significant pain reduction.

Is Freiberg’s disease the same as metatarsalgia?
No. Metatarsalgia is general ball-of-foot pain with many causes. Freiberg’s disease is a specific structural diagnosis — avascular necrosis confirmed by imaging. Misdiagnosis leads to ineffective treatment.

Early-stage Freiberg’s can revascularize with adequate offloading. Advanced stages with joint collapse generally do not remodel to normal.
