Gout: Acute Care + Long-Term Management

Gout is caused by uric acid crystal deposits in joints, most classically the big toe joint (first metatarsophalangeal joint). It requires two types of care: acute management of painful flares (podiatry’s domain) and long-term uric acid reduction (internal medicine’s domain). At Balance Foot & Ankle, we provide both the acute foot care and coordinate with Vassallo Medical Group and other internists for the systemic management that prevents recurrence.

Gout in the Foot: What We Treat

During a gout flare, the affected joint becomes exquisitely painful, swollen, red, and hot — often so sensitive that even a bed sheet causes agony. We provide acute gout care including: accurate diagnosis (joint aspiration when needed to confirm urate crystals), prescription anti-inflammatory management, joint offloading to protect the affected joint during the flare, and MLS laser therapy which reduces acute joint inflammation and provides rapid pain relief.

Chronic Gout Foot Management

Recurrent gout leads to tophi (uric acid deposits under the skin), chronic joint damage, and progressive arthritis. Custom orthotics protect damaged joints and reduce load on affected metatarsals. Surgical consultation may be needed for large tophi or advanced joint destruction.

During an acute gout attack, the affected joint is exquisitely tender, red, swollen, and warm — most patients cannot bear to have even a bedsheet touch the affected area.
