Why Swelling Develops After Heart Surgery
Cardiac surgery involves the thoracic vessels and bypass circuits that temporarily alter the fluid dynamics of the entire vascular system. Post-operative inflammatory responses increase capillary permeability. Reduced mobility during recovery allows fluid to pool in the lower extremities. In bypass procedures using leg veins (saphenous vein harvesting), the altered venous anatomy of the leg can cause persistent edema on that side.

Swollen Feet After Heart Surgery in Michigan: Edema Management for Cardiac Surgery Patients
Lower extremity edema following cardiac surgery — coronary artery bypass grafting (CABG), valve replacement, or other open heart procedures — is extremely common and presents specific foot care challenges that are best managed by a podiatrist experienced with the post-cardiac surgery population. The edema results from a combination of fluid resuscitation during surgery, postoperative inflammation, reduced cardiac output during recovery, and venous insufficiency from saphenous vein harvesting (used as bypass conduit in CABG) that alters venous return from the legs. The resulting bilateral lower leg and foot swelling creates tight, fragile skin that is highly susceptible to breakdown, infection, and venous stasis changes — and the immunosuppression or anticoagulation many cardiac surgery patients receive reduces the normal protective responses to minor foot trauma.
