The Foot Consequences Nobody Talks About

Smoking causes lung cancer. It causes heart disease. Most people know this. What most people don’t know is that smoking is also the single greatest risk factor for peripheral arterial disease — and PAD can lead directly to foot amputation. As a podiatrist, I see the foot consequences of smoking regularly. This is what cigarettes do to your feet.

How Smoking Damages Foot Circulation

Cigarette smoke contains over 7,000 chemicals. Nicotine causes immediate vasoconstriction — every cigarette you smoke restricts blood flow to the feet for up to 90 minutes. Long-term, smoking accelerates atherosclerosis (plaque buildup in arteries), causes chronic inflammation of blood vessel walls, reduces oxygen-carrying capacity of the blood, damages the endothelial lining of arteries, and promotes blood clotting. The result: narrowed, damaged arteries that can’t deliver adequate blood to the feet.

Smoking and PAD: The Numbers

Smokers are 2–4 times more likely to develop peripheral arterial disease than non-smokers. Among patients with PAD severe enough to require surgery, smoking is responsible in the vast majority of cases. The risk is dose-dependent: heavy smokers and long-term smokers have far higher rates of severe PAD than light or occasional smokers.

What This Means for Your Feet

When PAD develops in the feet and legs, wounds heal poorly — a small blister or cut that would heal in days for a non-smoker may take weeks or fail to heal entirely. Infections develop more easily and spread faster. In the most severe cases, gangrene develops and amputation becomes necessary. Smoking-related PAD is the leading cause of non-traumatic lower limb amputation in the United States.
