How Stress Fractures Develop

Distal Fibula: Common in runners. Presents as lateral ankle pain and is often confused with a sprain. Responds well to boot and activity modification.

Diagnosis

Plain X-ray is insensitive in the first 2-4 weeks — MRI is the gold standard for early diagnosis and site characterization. Bone scan is an alternative when MRI is unavailable.

Treatment and Recovery

Low-risk fractures (metatarsals 2-4, calcaneus, fibula): Protected weight bearing in a CAM boot for 6-8 weeks with gradual return to activity. Most athletes return to full sport in 8-12 weeks.

High-risk fractures (navicular, Jones, medial malleolus, anterior tibial cortex): Often require non-weight bearing casting or surgical fixation. Return to sport may take 3-6 months.

Nutritional optimization is critical — calcium, vitamin D, and adequate caloric intake support bone healing. Address any underlying RED-S (Relative Energy Deficiency in Sport) in female athletes.
