In our clinic, I stage bunions by the hallux valgus angle (HVA) on weight-bearing X-ray plus the patient’s functional complaint. The stage determines whether conservative care is a finish-line or just a bridge to surgery.

Stage 1 — Mild (HVA 15-20°)

The big toe drifts but the joint is still mobile and the bump is cosmetic more than painful. Wide-toe-box shoes, a daytime spacer, and an orthotic to slow progression are the entire treatment. Surgery at this stage is almost never indicated. This is where 50% of my bunion patients live for 15+ years without progression if they’re disciplined about footwear.

Stage 2 — Moderate (HVA 20-40°)

This is where pain starts showing up—bursitis on the bump, transfer metatarsalgia under the 2nd/3rd heads, and shoe-fit problems. Conservative care still works for 60-70% of patients but requires the full 12-product stack below plus 6-8 weeks of discipline. If conservative care fails here, we discuss osteotomy. Weeks 1-2: Unload + Assess

The first two weeks are about stopping the daily micro-insults. Switch to a wide-toe-box trainer (products #1 and #4 below) for every waking hour. Dr. Tom’s Clinical Tip: The critical cue is “just proximal to the metatarsal heads”—that means between the heads and the middle of the foot, not directly under the heads. If the pad is under your ball-of-foot pain, it’s in the wrong spot. Move it 1cm back toward the heel.

Karen came in to our Howell office last September with a Stage-2 bunion on her left foot. Hallux valgus angle: 32 degrees. Wearing narrow ballet flats to work 9 hours a day. In Our Clinic

In our clinic, bunion patients come in at two very different stages. The first group is women in their 30s and 40s noticing a small bump and seeking nonsurgical slowing tactics — wide toe box shoes, bunion splints at night, custom orthotics to redistribute load away from the first MTP. The second group is patients in their 50s+ who can no longer find shoes that fit and are asking, honestly, about surgery. Our standard workup includes weight-bearing X-rays to measure the intermetatarsal angle and the HVA. Patients with an IMA under 13° usually do well conservatively; 13°+ often benefits from a surgical plan.

FAQ

Can a night splint actually reverse my bunion?

No. The bone deformity is permanent once formed. But the splint can lengthen the tight medial capsule and reactivate the abductor hallucis muscle, which often produces meaningful functional improvement even with an unchanged HVA on X-ray. I tell patients: measure your win in pain and function, not in X-ray angle.

How do I know if I need surgery vs. conservative care?

Try honest conservative care for 12 weeks first—the full stack above, not just one product. If your pain is still 6+/10 and the HVA is over 40 degrees, we discuss osteotomy. If you’re better than 4/10 at week 12, keep going—surgery has a 6-12 month recovery and is worth avoiding if function is adequate.

Are bunion correctors from Amazon a scam?

Most of them overstate what they do. None will reverse the bone angle. But a well-designed night splint (#3) and daytime spacer (#5) have real mechanical benefits—not correction, but protection and stretch. The scam is the language, not the category.

Can I still run with a bunion?

Yes, in a wide-toe-box trainer (#2) with the orthotic (#1). I run and I have early hallux valgus. The mistake is running in narrow road shoes—that accelerates everything. Switch the shoe first, then keep running.

What’s the difference between PowerStep Pinnacle vs. Pinnacle Maxx?

Will insurance cover custom orthotics instead of these?

How long does the whole protocol take to work?
