When Does Insurance Cover Flat Foot Treatment?

The key rule for flat foot (pes planus) insurance coverage: symptoms matter. Insurance covers treatment when flat feet cause documented pain, gait abnormality, functional limitation, or joint overuse injuries linked to arch collapse. Flat feet that are found incidentally with no symptoms are not covered for treatment — and that’s appropriate, since asymptomatic flat feet often require no treatment. Flat Foot Surgery Coverage

Surgical flat foot reconstruction (various procedures depending on severity and structure — tendon transfers, calcaneal osteotomy, joint fusions) is covered with pre-authorization when conservative treatment has failed and there is documented pain and functional impairment. Adult acquired flatfoot (AAFD) from posterior tibial tendon dysfunction is among the most commonly authorized flat foot surgical diagnoses. Pediatric flexible flat feet are generally managed conservatively and rarely require surgical authorization.

Stages of Flat Foot and Coverage Implications

PTTD (posterior tibial tendon dysfunction) is staged I–IV, with treatment and coverage varying by stage. Stage I: conservative treatment covered. Stage II: conservative treatment; surgical coverage when conservative fails. Stage III–IV: surgical reconstruction often the only effective treatment; covered with pre-authorization. My flat feet don’t hurt — should I get them treated? Generally, no. Asymptomatic flat feet that don’t cause pain or functional problems often don’t need treatment. If you have concerns, we can evaluate and advise. But insurance correctly doesn’t cover treating conditions that aren’t causing problems.

My child has flat feet — is that covered? Pediatric flexible flat feet are common and usually resolve with age. We evaluate pediatric flat feet conservatively. When custom orthotics are clinically indicated for a symptomatic child, insurance coverage follows the same rules as adults — medical necessity must be documented.
