How Children’s Feet Develop
Children’s feet are not miniature adult feet — they are cartilage-rich structures in the process of ossification and development. The arch develops gradually between ages 3–8. The bones are softer and more susceptible to developmental deformity from improper footwear. By age 18, the foot has reached full skeletal maturity.
Shoe Fitting for Children
Growing room: 0.5 inch (1.25 cm) between the longest toe and shoe end. Children’s feet grow 0.5–1 shoe size every 3 months in toddlers, slowing to every 4–6 months in older children.
Width: Wide enough for the toes to spread comfortably. Squeeze the widest part — if the shoe bulges, it’s too narrow.
Flexibility: Shoes should bend at the ball of the foot (where the foot naturally bends) but be firm at the arch and heel.
Fastenings: Velcro for young children who can’t yet tie laces; lace-up for older children for better adjustability.
Replace frequently: Check fit every 3 months — children’s feet outgrow shoes quickly.
Common Childhood Foot Conditions
Sever’s disease (calcaneal apophysitis): Heel pain in active children ages 8–14 from inflammation at the growth plate. Responds to rest, heel lifts, calf stretching, and reduced sports load.
Flat feet: Normal in toddlers; flexible flat feet that are painless usually don’t require treatment. Painful flat feet, rigid flat feet, or flat feet causing gait problems warrant evaluation.
In-toeing / out-toeing: Usually resolves spontaneously; persistent or unilateral cases need evaluation.
Plantar warts: Very common in school-age children using communal pools. Contagious — treat promptly to prevent spread.
Toenail Care for Children
Trim straight across — not curved — to prevent ingrown toenails. Trim after bathing when nails are softer. Seek podiatric evaluation for any ingrown toenail that is painful, red, or draining.
