The Most Common Hiking Foot Injuries

Blisters: The most common hiking foot problem. Caused by friction, moisture, and heat — exacerbated by ill-fitting boots, cotton socks, and new footwear without adequate break-in. Prevention: merino wool or synthetic moisture-wicking socks (never cotton), correctly fitted boots (thumb-width of space at the toe; snug heel without lift), and gradual mileage build-up with new footwear.

Plantar fasciitis: Uneven terrain creates repetitive arch stress that exceeds the plantar fascia’s capacity, particularly on downhill sections where the foot slaps flat with each step. Most common in hikers who rapidly increase mileage (weekend warriors). Prevention: arch-supportive insoles in hiking boots and progressive mileage increases.

Metatarsal stress fractures: The navicular, second, and third metatarsals are the most common stress fracture sites in hikers — particularly in long-distance trekkers (through-hikers, Camino pilgrims) who dramatically increase weekly mileage. Early signs: deep aching in the midfoot or forefoot that worsens with each day’s mileage. Requires rest and sometimes immobilization.

Choosing Hiking Boots and Insoles

Hiking boot selection: match boot to terrain. Low-cut trail runners for well-maintained trails with light packs. Mid or high-cut boots for rugged terrain, off-trail, or heavy packs (ankle support matters with 35+ lb loads). Ensure there is adequate toe room — feet swell during long hikes, and toe boxes that fit perfectly in-store may create pressure after 10 miles.
