The Evolution of Bunion Surgery
Bunion correction surgery has evolved dramatically over the past decade. Traditional open bunion surgery — effective for decades — requires relatively large incisions, more extensive soft tissue dissection, and recovery times measured in weeks of non-weight bearing followed by months of gradual return to normal shoes. While outcomes are generally excellent, the recovery burden deters many patients with painful bunions from pursuing the surgical correction they need.
Minimally invasive bunion surgery — including the MICA (Minimally Invasive Chevron and Akin) technique and its variations — has emerged as a transformative alternative. Through small stab incisions no larger than 3 to 4 millimeters, specialized burrs and instruments reshape the bone under fluoroscopic X-ray guidance. Most patients can bear weight in a postoperative shoe immediately after surgery, with return to normal footwear by 6 to 8 weeks.

What Is the MICA Technique?
The MICA technique combines a minimally invasive distal metatarsal chevron osteotomy (a V-shaped bone cut at the head of the first metatarsal) with an Akin osteotomy (a closing wedge cut at the base of the proximal phalanx) to correct the three-dimensional deformity components of a hallux valgus bunion. The entire procedure is performed through 3 to 4 small stab incisions using specialized burrs operating under real-time fluoroscopic (X-ray) imaging.
The metatarsal head is shifted laterally to correct the widened intermetatarsal angle, the great toe is realigned, and small titanium headless compression screws are inserted percutaneously to hold the corrected position during bone healing. The small incisions allow primary closure with a single suture each and reduce the soft tissue trauma that drives the swelling and prolonged recovery of traditional open surgery.
Candidacy for Minimally Invasive Bunion Surgery
Minimally invasive techniques are appropriate for a broad range of bunion severity. Mild to moderate bunions (intermetatarsal angles up to approximately 18 to 20 degrees) are reliably correctable with the MICA technique. Moderate to severe bunions may require modified approaches or supplemental procedures. Very severe bunions with significant first metatarsal instability — those that are most effectively treated with the Lapidus procedure (first tarsometatarsal joint fusion) — may not be candidates for isolated MICA correction.
Ideal candidates are patients with symptomatic bunions causing pain, shoe-fitting difficulty, or activity limitation who have adequate bone density, no active infection, and reasonable soft tissue quality. Age is not a limiting factor — both younger adults seeking return to athletic activity and older adults seeking comfortable daily function achieve excellent results.
Patients with significant arthritic changes at the first metatarsophalangeal joint, prior first metatarsal surgery, or significant first metatarsal hypermobility may be better served by other surgical approaches. Our surgeons perform comprehensive preoperative evaluation including weight-bearing X-rays and clinical assessment to determine the optimal procedure for each patient.
The Surgical Procedure
MICA is performed under regional or general anesthesia as an outpatient procedure. The patient is supine with the operative foot slightly elevated on the operating table. The C-arm fluoroscope is positioned to provide real-time AP and lateral views of the foot throughout the procedure.
Small stab incisions are made at planned burr entry points — typically at the medial first metatarsal head, the first web space, and the medial proximal phalanx for the Akin component. Specialized small-diameter burrs (2 to 3mm) powered by an electric handpiece create the precise bone cuts under continuous fluoroscopic visualization. The metatarsal head is shifted laterally and slightly plantarward, correcting the intermetatarsal angle and restoring the normal metatarsal head position.
Two to three headless compression screws are inserted percutaneously through additional small puncture sites to secure the osteotomy in the corrected position. The Akin proximal phalanx wedge is performed similarly with a single percutaneous screw for fixation. The stab wounds are irrigated and closed with single sutures or steri-strips. A soft dressing and bunion bandage is applied and the patient is placed in a postoperative flat shoe.
Recovery Advantages of Minimally Invasive Surgery
Immediate weight bearing is the most significant practical advantage of MICA compared to traditional open bunion surgery. Most patients walk out of the surgical center in a flat postoperative shoe within hours of surgery. The dramatically smaller incisions produce less post-operative swelling, less bruising, and less soft tissue pain than open approaches.
The typical recovery timeline includes weight bearing in the flat postoperative shoe for 6 to 8 weeks while bone healing occurs. Transition to a wide sneaker or athletic shoe begins when X-rays confirm adequate healing at the osteotomy sites. Most patients return to standard footwear including narrower dress shoes by 3 to 4 months. Return to athletic activities including running and court sports is typically achieved by 4 to 6 months.
Swelling is the limiting factor in return to normal footwear and activity — the foot continues to show some degree of swelling for 6 to 12 months even when pain has resolved and bone healing is complete. This is normal and resolves completely with time.
Comparison to Traditional Open Bunion Surgery
Both MICA and traditional open techniques (distal chevron osteotomy, Scarf osteotomy, Austin procedure) achieve excellent deformity correction and patient satisfaction in appropriately selected patients. The primary practical difference is the recovery profile. Traditional open surgery typically requires 2 to 4 weeks of limited weight bearing and 3 to 6 months before return to normal footwear; MICA reduces this substantially with immediate weight bearing and 6 to 8 weeks in the postoperative shoe.
For very severe bunions or those with first tarsometatarsal joint instability requiring the Lapidus procedure, open surgery remains the gold standard because the Lapidus requires rigid internal fixation of the joint fusion that benefits from the direct visualization of open technique. These cases are identified during preoperative evaluation and the patient is counseled accordingly.
Combining Bunion Correction with Other Procedures
Bunion correction is frequently combined with hammertoe correction, sesamoidectomy, or lesser metatarsal osteotomies when concurrent pathology is present. These additional procedures can typically be performed through the same minimally invasive portals during the same operative session, providing comprehensive forefoot reconstruction without additional surgical morbidity.

In our clinic, bunion patients come in at two very different stages.
