Why Salicylic Acid Works
Salicylic acid is a keratolytic — it breaks down the protein bonds holding dead skin cells together. Applied to a plantar wart, it gradually removes the thickened overlying callus and the wart tissue itself, layer by layer. It also has a mild irritant effect on the underlying skin that may stimulate local immune recognition of HPV antigens exposed by the keratolysis — contributing to immune-mediated clearance.
The Correct Protocol (Most Failures Skip Step 2)
Step 1 — Soak: Soak the foot in warm water for 10–15 minutes to soften the callus overlying the wart.
Step 2 — Pare (the critical step): Using an emery board or pumice stone, file away the white, dead skin covering the wart. You’re trying to reach the actual wart tissue (small brown or black dots — thrombosed capillaries — visible within). Without this step, the acid does not penetrate to live wart tissue and is far less effective.
Step 3 — Dry completely: Pat foot dry. Allow to air dry 5 minutes. Salicylic acid works poorly on wet skin.
