Mechanism of Action

Shockwaves are high-energy acoustic pulses generated by a focused or radial pressure wave generator that are transmitted through the skin to the target tissue. Unlike therapeutic ultrasound, which operates at continuous or pulsed low intensity, ESWT delivers very high-pressure pulses (peak pressure 10–100 MPa) at low repetition rates. The biological effects include: cavitation (micro-bubble formation and collapse that disrupts pathological calcifications); neovascularization (stimulation of angiogenesis at the treatment site through upregulation of VEGF and other growth factors); neuromodulation (temporary reduction in Substance P and pain signal transmission producing the analgesic effect); and mechanotransduction (stimulation of fibroblast activity and collagen remodeling in tendinopathic and fasciopathic tissue).

Evidence for Plantar Fasciitis

ESWT for chronic plantar fasciitis has the strongest evidence base of any foot application. Multiple Level 1 randomized controlled trials and systematic reviews demonstrate consistent superiority to placebo and comparable or superior outcomes to corticosteroid injection at 12-month follow-up. The American College of Foot and Ankle Surgeons (ACFAS) clinical consensus statement recommends ESWT as a non-surgical option after failure of 3+ months of conservative care. High-energy protocols (single session at ≥0.25 mJ/mm²) have slightly faster onset; low-energy protocols (3 sessions at lower energies) produce equivalent long-term results with better tolerability.

Evidence for Achilles Tendinopathy

ESWT for mid-portion Achilles tendinopathy has Level 1 evidence for both insertional and mid-portion types. For insertional Achilles tendinopathy — one of the most challenging conditions in podiatric practice — ESWT offers a non-surgical option superior to eccentric exercise alone and has the significant advantage of not carrying corticosteroid injection’s tendon rupture risk at the insertion site.
