The lateral foot contains the fifth metatarsal, cuboid bone, peroneal tendons (longus and brevis), and the sural nerve. Pain in this region can arise from any of these structures, and the precise location of pain is a key diagnostic clue.
Most Common Causes
Peroneal tendonitis: Aching pain behind the lateral malleolus extending toward the fifth metatarsal, worsened by activity. Common in runners and following ankle sprains. Cuboid syndrome: Lateral midfoot pain at the cuboid bone from subluxation or peroneal longus irritation — often called “the forgotten diagnosis.” Fifth metatarsal fracture (Jones fracture or avulsion): Sharp pain at the base of the fifth metatarsal after a twisting injury or overuse.
