What Are Plantar Warts?

Plantar warts (verruca plantaris) are benign skin growths on the sole of the foot caused by human papillomavirus (HPV) infection. The virus infects the superficial layers of the skin (epidermis) through micro-abrasions — common in community swimming pools, locker rooms, and any barefoot environment where the virus persists. They are distinguished by their characteristic appearance (pinpoint black dots from thrombosed capillaries, interrupted skin lines) and their location — ground pressure drives them inward, creating a callus surface.

Why Home Treatments So Often Fail

Salicylic acid products (the active ingredient in most over-the-counter wart treatments) work by chemically exfoliating the wart tissue — but the virus lives in deeper skin layers not always reached by topical agents. Success rates for OTC treatments are modest at best (approximately 40-50% after 12+ weeks of consistent use). Common reasons for failure: insufficient concentration, inadequate skin penetration, inconsistent application, and failure to fully debride the overlying callus before each treatment application. Deep or mosaic warts (clusters of multiple adjacent warts) are particularly resistant to topical treatment.

Professional Treatment Options

Cryotherapy (liquid nitrogen): freezing destroys wart tissue and stimulates an immune response. Multiple treatments 2-3 weeks apart are typically required. Canthardin: a blister-inducing agent applied professionally that destroys the wart through a blister response — painless at application but produces a blister within 24-48 hours. Bleomycin injection: directly injected into the wart, bleomycin destroys HPV-infected cells with high clearance rates. Pulsed-dye laser: destroys wart vasculature, starving the wart. Surgical excision: direct removal under local anesthesia for resistant single warts. Immunotherapy (topical Candida antigen, squaric acid): stimulates the immune system to recognize and attack HPV.

Prevention of Plantar Warts

Wear footwear in communal wet environments (pools, locker rooms, hotel showers). Avoid touching warts on others or your own. Inspect feet regularly and treat any wart promptly — they are much easier to eliminate when small. Keep feet dry — the virus thrives in moist environments.
