What Are the Sesamoids?
The sesamoid bones are two small bones embedded within the flexor hallucis brevis tendon, located on the ball of the foot directly beneath the first metatarsal head (at the base of the great toe). They function like kneecaps for the great toe — providing mechanical advantage for the flexor tendon, protecting the tendon from friction, and bearing significant weight with each push-off. Because of this load, the sesamoids are vulnerable to both acute fractures and chronic inflammatory conditions.
Sesamoiditis vs. Sesamoid Fracture
Sesamoiditis (inflammation of the sesamoid bones and surrounding tissues) is more common than true sesamoid fracture. Both cause pain under the first metatarsal head — the distinction matters because treatment differs. Sesamoiditis develops gradually from repetitive loading; sesamoid fracture typically has an acute mechanism (high-impact landing, direct trauma). Adding to diagnostic complexity: the medial sesamoid is a bipartite bone (naturally divided into two pieces) in approximately 10-25% of the population — bipartite sesamoid can be mistaken for fracture on X-ray. The smooth, rounded edges of a bipartite sesamoid versus the jagged edges of an acute fracture help distinguish them.
Diagnosis
Clinical examination reveals pinpoint tenderness directly under the first metatarsal head, worsening with passive great toe dorsiflexion (which stretches the sesamoid complex). Weight-bearing X-rays evaluate for fracture, fragmentation, and avascular necrosis (AVN — loss of bone blood supply causing collapse). Bone scan or MRI confirms sesamoid stress reactions not visible on X-ray and identifies AVN. Tibial versus fibular sesamoid involvement and whether the condition is unilateral or bilateral affect treatment planning.
Treatment
Sesamoiditis treatment begins with offloading: custom orthotics with a first metatarsal cutout (recessing the area under the sesamoids) and a Morton’s extension (stiffening the hallux MTP joint) dramatically reduce sesamoid loading. Carbon fiber insoles provide stiffness in regular footwear. Activity modification reduces aggravating loading. Anti-inflammatory measures (ice, NSAIDs) address acute inflammation. Corticosteroid injection into the first MTP joint provides relief when conservative measures are insufficient. Sesamoid fractures require non-weight-bearing in a boot for 6-8 weeks or longer. Surgical sesamoidectomy (removing the affected sesamoid) is a last resort for chronic, refractory cases — it alters great toe biomechanics and is avoided when possible.
Prevention for Athletes
Dancers, gymnasts, runners, and any athlete with high first MTP joint loading should incorporate sesamoid-protective footwear and orthotic strategies before pain develops. Any persistent ball-of-foot pain under the big toe warrants podiatric evaluation — early conservative management prevents the progression to chronic conditions requiring prolonged treatment.
