Soft tissue masses of the foot and ankle — the majority of which are benign — are a common presenting complaint in podiatric practice. Patient anxiety about foot masses is understandable, but most foot masses (ganglion cysts, lipomas, plantar fibromas) are completely benign, and distinguishing them from the rare malignant soft tissue tumor requires systematic evaluation rather than reflexive excision. Understanding the characteristic features of the most common benign foot masses guides appropriate management.
Ganglion Cysts
The most common soft tissue mass in the foot and ankle — a ganglion cyst is a benign myxoid cyst arising from a joint capsule or tendon sheath and containing viscous mucinous fluid. Common locations in the foot: dorsal midfoot (over the tarsometatarsal joints — the most common location); dorsal ankle (over the anterior ankle joint); plantar foot (from the plantar fascia or flexor tendon sheath); the sinus tarsi (arising from the subtalar joint). Clinical features: smooth, mobile, firm or fluctuant mass; transilluminates with a penlight (distinguishes cyst from solid mass); may wax and wane in size; causes pain from pressure on adjacent nerves or tendons; may restrict footwear. Imaging: ultrasound (the most efficient imaging — confirms cyst characteristics, identifies connection to joint or tendon sheath); MRI for surgical planning of complex or deep cysts. Treatment: observation (many ganglions resolve spontaneously — recommended for asymptomatic cysts); aspiration (relieves symptoms immediately but 50% recurrence rate); surgical excision with removal of the stalk at its joint or tendon sheath origin (10–15% recurrence with complete stalk removal).
Lipoma
A lipoma is a benign mass of mature adipocytes enclosed in a thin fibrous capsule. Foot and ankle lipomas are less common than in the trunk and proximal extremities but produce symptoms when located beneath the plantar fascia (plantar foot pain mimicking plantar fasciitis), along the tarsal tunnel (producing tarsal tunnel syndrome by compressing the posterior tibial nerve), or in the deep plantar space (producing mass effect on flexor tendons). Clinical features: soft, doughy, mobile mass; rarely tender unless compressing adjacent nerve or tendon; non-transilluminating (distinguishes from ganglion); slow-growing. MRI confirms lipomatous tissue (uniform fat signal on T1). Surgical excision is curative; recurrence is uncommon with complete excision.
Plantar Fibroma
A plantar fibroma (plantar fibromatosis, Ledderhose disease) is a benign fibroblastic nodule arising within the plantar fascia substance, most commonly in the medial band. The plantar fascia becomes nodular with one or multiple firm, non-mobile nodules palpable in the plantar arch. Unlike ganglion cysts, plantar fibromas are not cystic and do not transilluminate. Bilateral in 25% of cases and associated with Dupuytren’s contracture of the hand and Peyronie’s disease. Treatment: orthotics with plantar fibroma accommodation (cutout beneath the nodule) for symptomatic relief; intralesional corticosteroid injection (may reduce size but rarely eliminates); verapamil gel (has some evidence for size reduction); surgical excision — requires wide excision of the entire involved plantar fascia segment to minimize recurrence; local recurrence rate is significant (20–50%) with limited excision.
