Accessory Navicular Treatment Michigan
An accessory navicular is an extra bone (ossicle) located on the medial (inner) side of the foot, adjacent to the navicular bone near the arch. Present in approximately 10–14% of the population, it is usually asymptomatic — but in some individuals, the accessory bone becomes painful from direct pressure against shoes, or from the attachment of the posterior tibial tendon to the accessory rather than the main navicular bone. Dr. Tom Biernacki DPM at Balance Foot & Ankle treats accessory navicular pain in Michigan at our Howell, Brighton, and Bloomfield Hills locations.
Types of Accessory Navicular
- Type I: Small sesamoid bone within the posterior tibial tendon; rarely symptomatic
- Type II: Larger accessory ossicle connected to the navicular by a fibrocartilaginous synchondrosis; most commonly symptomatic. The posterior tibial tendon inserts partially on the accessory bone rather than the main navicular, reducing tendon pull efficiency.
- Type III: The accessory bone has fused with the main navicular, forming a “cornuate navicular” — a prominently enlarged navicular tuberosity.
Symptoms
- Prominent, palpable bony bump on the inner side of the arch
- Pain and tenderness over the navicular prominence
- Redness and callus formation from shoe irritation
- Arch pain and fatigue — especially in flatfoot
- Symptoms often begin in adolescence with increased activity
Treatment
Conservative treatment: Immobilization in a CAM boot for 4–6 weeks reduces acute inflammation at the synchondrosis. Custom orthotics provide medial arch support and offload the prominence. Wide medial footwear relief in the shoe prevents direct pressure. Ice and anti-inflammatory medications manage flares.
Surgical treatment (Kidner procedure) is offered for symptomatic Type II accessory navicular that has failed 3–6 months of conservative care. Dr. Biernacki excises the accessory bone through a medial arch incision and advances the posterior tibial tendon to its anatomic insertion on the main navicular, restoring tendon mechanics and arch support. Recovery: non-weight-bearing × 4 weeks, boot × 4 weeks, return to full activity at 3–4 months. Patient satisfaction exceeds 85%.

Frequently Asked Questions
Does an accessory navicular cause flat feet?
Type II accessory navicular can contribute to flatfoot because the posterior tibial tendon inserts partially on the accessory bone rather than its normal attachment, reducing arch support. The Kidner procedure restores anatomic tendon attachment and can improve arch height.
How do I know if I have an accessory navicular?
A bony prominence on the inner side of the arch that is tender to direct palpation is characteristic. Weight-bearing X-rays confirm the diagnosis, showing the accessory ossicle adjacent to the navicular. Dr. Biernacki can diagnose this at your first visit.
