Toe fractures are among the most common foot injuries — most result from stubbing the toe against furniture, dropping something on the foot, or athletic collisions. While many toe fractures heal well with simple buddy taping, certain fractures require more aggressive treatment to prevent chronic pain and deformity. Which Toe Is Broken?

Big toe (first toe) fractures are more serious and require more aggressive management than lesser toe fractures because the big toe bears significant weight during walking and pushoff. Displaced or intra-articular fractures often require surgical fixation.

Lesser toe (2nd–5th) fractures most commonly occur at the distal phalanx or proximal phalanx of the smaller toes. The majority are non-displaced and heal well with buddy taping and a stiff-soled shoe in 4–6 weeks. Displaced fractures and those involving the joint surface may require reduction or surgery.

Treatment

Buddy taping — taping the injured toe to the adjacent toe — is effective for most non-displaced lesser toe fractures. It provides splinting support while allowing walking. Padding between toes prevents maceration. Change the tape every 2–3 days.

Stiff-soled shoe or surgical shoe protects the toe during healing by preventing the toe from bending during walking.

Reduction (straightening of a displaced fracture) is performed under digital nerve block when the toe is angulated or rotated. Most reductions are stable and maintained with buddy taping.

Surgical fixation with a pin or screw is required for unstable, displaced first toe fractures, intra-articular fractures with step-off, and fractures that cannot be adequately reduced by closed manipulation.
