How do I know if it’s gout or pseudogout?
Location is the best clinical clue: gout attacks the big toe (1st MTP joint) in 75% of cases; pseudogout most commonly attacks the knee and wrist. Age matters too — pseudogout is more common over 60. Definitive distinction requires joint fluid analysis showing crystal type under polarized microscopy. Serum uric acid is elevated in gout but normal in pseudogout.
How long does a gout attack in the foot last?
Untreated: 3–10 days. With appropriate anti-inflammatory treatment (NSAIDs or colchicine started early): often 24–48 hours. Colchicine is most effective when started within the first 12–24 hours of symptoms. The joint may remain tender and slightly swollen for 1–2 weeks after the acute attack resolves.
What foods trigger gout attacks?
High-purine foods: organ meats (liver, kidney, sweetbreads), red meat, shellfish (shrimp, lobster, anchovies), alcohol (especially beer and spirits), and high-fructose corn syrup (sodas, packaged juices). Dehydration amplifies urate concentration. Low-fat dairy (yogurt, skim milk) is actually protective — it increases uric acid excretion.
Can gout be cured?
Gout attacks can be eliminated with consistent uric acid-lowering therapy (allopurinol, febuxostat). When serum uric acid is maintained below 6 mg/dL for years, existing tophi dissolve and new attacks do not occur. In that sense, gout is one of the only forms of arthritis that can be effectively cured long-term with consistent medication.
