When the fat pad atrophies from age, diabetes, corticosteroid injection, or mechanical wear, heel impact forces are transmitted directly to the calcaneal periosteum and the heel skin, producing a deep, aching pain under the heel that is distinct from plantar fasciitis and responds poorly to plantar fasciitis treatments. Pathophysiology and Diagnosis
