The foot, ankle, knee, hip, and spine function as a linked kinetic chain—mechanical dysfunction at one level transmits forces up (or down) the chain and can produce symptoms at distant joints. The foot is the foundation of this chain: every step begins with foot contact, and the way the foot absorbs and transmits force directly affects the mechanics of the ankle, knee, and hip above it. Podiatrists evaluate the foot-knee relationship routinely because addressing the root cause in the foot can resolve knee pain that has failed to improve with knee-focused treatments.
Overpronation and Patellofemoral Pain
Overpronation—excessive inward rolling of the foot during the stance phase of gait—is the most commonly cited foot-related contributor to knee pain. When the foot overpronates, the tibia internally rotates, which causes the femur to internally rotate and the patella (kneecap) to track laterally relative to the femoral groove. This malalignment creates abnormal stress on the patellofemoral joint, producing anterior knee pain (pain around or behind the kneecap) with activities like walking, running, stair climbing, and sitting for prolonged periods.
