Plantar plate tears — disruption of the fibrocartilaginous plantar plate at the lesser metatarsophalangeal (MTP) joint — are a frequently underdiagnosed cause of forefoot pain, progressive lesser toe deformity, and the crossover toe deformity. The plantar plate is the primary stabilizer of the MTP joint against dorsal subluxation; when it fails, the toe progressively migrates dorsally and medially — producing the crossover second toe deformity that is essentially pathognomonic for second MTP plantar plate disruption.

Anatomy and Clinical Presentation

The plantar plate — a dense fibrocartilaginous structure attached to the plantar base of the proximal phalanx and the deep transverse metatarsal ligament — resists MTP joint hyperextension and bears metatarsal head plantar pressure during push-off. Predislocation syndrome (the clinical precursor to frank dislocation): plantar forefoot pain localized to the second MTP joint, pain with MTP dorsiflexion, swelling of the second MTP joint (a plantar effusion visible on physical examination), and positive drawer test (Lachman test of the MTP joint — longitudinal traction on the proximal phalanx producing dorsal subluxation indicates plantar plate incompetence). MRI of the forefoot confirms plantar plate integrity and grades the tear — grade 0 (attenuation without tear) through grade 3 (complete disruption with dislocation).

Treatment

Conservative management for grade 0–1 tears: metatarsal pad placed proximal to the metatarsal head to offload the MTP joint, dorsal MTP taping (figure-8 tape holding the toe in slight plantarflexion), and modification of footwear with a wide toe box and low heel. Surgical management for grade 2–3 tears and failed conservative management: direct plantar plate repair through a plantar approach, frequently combined with Weil metatarsal shortening osteotomy to reduce tension on the repair site.
