The proliferation of advanced wound care dressings has created a complex decision landscape — with hundreds of products across multiple categories, selecting the appropriate dressing for a specific wound type is essential for optimal healing. The fundamental principle guiding dressing selection: match the dressing to the wound’s phase of healing, exudate level, infection status, and anatomical location. Over-dressing a wound is as harmful as under-treating it.

Dressing Categories and Indications

Foam dressings: highly absorbent open-cell polyurethane foam — the workhorse dressing for moderate-to-heavy exudate wounds; maintains moist wound environment without maceration; available in adhesive (for wounds with intact periwound skin) and non-adhesive (for fragile periwound skin) forms; change every 2–4 days based on exudate level. Hydrocolloid dressings: gel-forming particles in an adhesive wafer that absorb light exudate and maintain the moist environment ideal for epithelialization — best for low-exudate, clean granulating wounds and superficial partial-thickness wounds; change every 3–5 days; characteristic brown gel is expected wound fluid, not infection. Alginate dressings: derived from seaweed; highly absorbent calcium alginate fibers that form a soft gel on contact with wound exudate — ideal for cavitating wounds with heavy exudate; requires a secondary dressing; do not use on dry wounds (will desiccate tissue). Silver-containing dressings (silver foam, silver alginate, Mepilex Ag, Aquacel Ag): broad-spectrum antimicrobial silver ions reduce bacterial bioburden — indicated for locally infected or critically colonized wounds; do not use on clean, non-infected wounds (silver ions are cytotoxic to healing cells at high concentrations); change every 3–7 days.

Non-adherent contact layers (Mepitel, Adaptic, Xeroform): placed directly on wound to prevent dressing adherence and trauma on removal — essential for fragile granulation tissue and epithelializing wounds before the secondary dressing. Collagen dressings: stimulate fibroblast activity in stalled wounds; most beneficial for chronic wounds >4 weeks without improvement. Honey-based dressings (Medihoney): osmotic and antimicrobial; useful for superficially infected and sloughy wounds.
