Running-related foot and ankle injuries account for 50–75% of all running injuries, and the majority are preventable — caused by training errors (too much volume or intensity increase too quickly) rather than biomechanical inevitability. Understanding the most common running injuries, the role of training load in their prevention, and evidence-based footwear principles allows runners to maintain high mileage with reduced injury risk.

Most Common Running-Related Foot Injuries

Plantar fasciitis: the most common running injury overall — caused by rapid mileage increases (>10% per week), inadequate rest, tight gastrocnemius-soleus, and excessive pronation. Prevention: gradual mileage increases; daily plantar fascia and gastrocnemius stretching; custom orthotics for hyperpronators. Metatarsal stress fractures: second metatarsal most common — caused by high mileage on hard surfaces, low bone density, female athlete triad. Prevention: calcium/vitamin D optimization; include rest days; vary running surfaces. Achilles tendinopathy: mid-portion in younger runners, insertional in masters athletes — caused by rapid mileage increases and hill running (increased Achilles tensile loading). Prevention: Alfredson eccentric protocol as a weekly maintenance exercise; gradual hill introduction. Ankle sprains: trail runners disproportionately affected — lateral ankle instability increases risk dramatically. Prevention: proprioceptive training with single-leg balance exercises.

Footwear Principles
