Athlete’s foot (tinea pedis) is a fungal infection caused by dermatophyte fungi (most commonly Trichophyton rubrum and T. mentagrophytes) that infect the stratum corneum of the skin. Dry skin (xerosis) is a moisture deficit condition with no infectious component. Both produce scaling, flaking skin on the feet—but they have distinct presentations, distributions, and treatments.

Classic athlete’s foot patterns: (1) interdigital—scaling, maceration, and fissuring between the toes, especially the third-fourth and fourth-fifth web spaces, often with itching and odor; (2) moccasin-type—scaling and fine peeling across the entire sole in a moccasin distribution, often mistaken for dry skin; (3) vesicular—small fluid-filled blisters on the arch and instep, often extremely itchy. Classic dry skin presentation: diffuse fine scaling without clear web space involvement, often associated with heel cracking, exacerbated by low humidity and hot water exposure.

The most helpful distinguishing feature: itching. Athlete’s foot typically produces significant itching, particularly in the web spaces. Dry skin is usually not itchy unless severely dehydrated and cracked. Web space involvement (scaling or maceration between toes) strongly suggests athlete’s foot rather than dry skin.

Treatment of Athlete’s Foot

Athlete’s foot responds to antifungal medication—not moisturizer. Using moisturizer on a fungal infection may worsen it by providing a moist environment fungi prefer. OTC antifungal options: clotrimazole 1% (Lotrimin), terbinafine 1% (Lamisil AT), miconazole 2% (Micatin). Apply to affected skin and 2 inches of surrounding tissue twice daily for 2–4 weeks—continue treatment for 1 week after symptoms resolve to prevent recurrence.

For moccasin-type tinea pedis (thick, chronic scaling of the entire sole), OTC treatment often fails because the thick scale prevents drug penetration. Prescription oral terbinafine or itraconazole for 2–6 weeks is typically required. Nail involvement (onychomycosis) virtually always accompanies moccasin-type tinea and requires oral treatment.

Prevention: dry feet thoroughly after showering (especially between toes), wear moisture-wicking socks, wear flip-flops in shared shower areas, rotate shoes to allow full drying between wears, use antifungal powder in shoes prone to sweating.

Treatment of Dry Skin

Dry skin responds to moisturization—not antifungals. The treatment hierarchy: (1) ammonium lactate 12% cream (prescription strength or OTC as Amlactin) is the most effective agent for thick scale and heel fissures; (2) urea-based creams (20–40% urea) provide both moisturization and keratolytic action dissolving thickened scale; (3) petroleum jelly (Vaseline) overnight with socks—the simplest and surprisingly effective treatment.

For cracked heels specifically: thick split heels are both a dry skin and mechanical problem—the heel bears weight and splits under load. Proper mechanical support (heel cups, orthotics) combined with hydration therapy produces faster resolution than hydration alone. Silicone heel cups reduce pressure that drives crack deepening.

When to suspect something else: if ‘dry skin’ doesn’t respond to 2–4 weeks of appropriate treatment, consider: tinea pedis (try antifungal), psoriasis (silvery scale often with nail changes), eczema (contact dermatitis—look for new shoe or sock material exposure), or onychomycosis spreading to surrounding skin.
