What Is Dactylitis?

Dactylitis refers to inflammation of a whole digit — not just a joint, not just the skin — causing diffuse swelling of the entire finger or toe that gives it a “sausage” appearance. In the foot, it typically affects one or more toes and involves the tendon sheaths (tenosynovitis), joints, and surrounding soft tissue simultaneously. Dactylitis is one of the classification criteria for psoriatic arthritis (PsA) and serves as an important diagnostic clue for inflammatory arthropathy.

Causes of Dactylitis

Psoriatic Arthritis (Most Common)

PsA causes dactylitis in approximately 30–40% of patients, typically early in the disease. Toes are slightly more commonly affected than fingers. It can occur with or without visible skin psoriasis. If you develop sausage toe without a known cause, PsA must be ruled out — early diagnosis and biologic treatment prevents joint destruction.

Reactive Arthritis

Reactive arthritis (formerly Reiter’s syndrome) develops 2–6 weeks after a GI or genitourinary infection (Chlamydia, Salmonella, Campylobacter). The triad of arthritis, urethritis, and conjunctivitis is classic but not always complete. Dactylitis — particularly affecting the toes — is common in reactive arthritis.

Gout

Acute gout flares most often affect the big toe joint (podagra), but can cause what appears to be dactylitis when the inflammatory response involves the entire toe with profound swelling and redness.

Infectious (Septic) Dactylitis

Bacterial infection within the flexor tendon sheath causes suppurative dactylitis — a medical emergency requiring urgent surgical drainage. Distinguish from inflammatory dactylitis by: rapid onset (<24 hours), severe pain with passive extension, fever, wound or puncture history. Requires emergency evaluation.

Diagnosis

Workup for a sausage toe includes: X-ray (periostitis, joint erosions), blood tests (CRP, ESR, uric acid, HLA-B27), skin exam for psoriatic plaques, nail examination, and often rheumatology referral. MRI of the affected toe reveals tenosynovitis, bone marrow edema, and joint involvement in inflammatory dactylitis. Synovial fluid analysis rules out infectious or crystal-induced causes.

Treatment

Treatment targets the underlying cause. NSAIDs provide short-term symptom relief across all inflammatory causes. For PsA: biologic therapies (TNF-alpha inhibitors, IL-17 inhibitors) rapidly resolve dactylitis and prevent joint erosion. For reactive arthritis: NSAIDs and treatment of the underlying infection; most cases resolve within 3–12 months. For gout: colchicine, NSAIDs acutely; allopurinol or febuxostat for long-term uric acid control. For infectious dactylitis: urgent surgical drainage and IV antibiotics — do not delay.

Is dactylitis the same as gout?

Not the same condition, but gout can cause what appears to be dactylitis — profound swelling of the entire toe during an acute attack, especially when the 1st MTP joint (big toe base) is involved. True dactylitis involves tenosynovitis of the flexor tendon sheath; gout involves crystal-induced synovitis within a joint. Blood uric acid and joint fluid crystal analysis distinguish them.

Can dactylitis go away on its own?

Reactive arthritis dactylitis often resolves over 3–12 months with NSAIDs and treatment of the triggering infection. Gout dactylitis resolves with anti-inflammatory treatment but recurs without uric acid management. Psoriatic arthritis dactylitis rarely resolves spontaneously and typically progresses without systemic biologic treatment. The underlying cause determines the natural history.

What does dactylitis look like?

The entire toe swells uniformly from the metatarsophalangeal joint (base) to the tip — hence “sausage toe.” Unlike joint-specific swelling (which is localized to one joint), dactylitis makes the whole digit appear puffy and cylindrical. The skin overlying the toe may look taut and shiny. There is usually tenderness along the full length of the toe, not just at one point. Redness varies — it’s more prominent in gout and infectious causes than in PsA.
